探讨早期微量喂养联合肠外营养治疗对ICU早产儿胃肠道功能及生长发育水平的影响

摘　　要：目的探讨早期微量喂养联合肠外营养治疗对ICU早产儿胃肠道功能及生长发育水平的影响。方法将2021年1月—2023年1月在本院ICU治疗的60例早产儿随机分为两组,对照组使用肠外营养,观察组使用早期微量喂养联合肠外营养,对比两组的胃肠功能恢复指标、营养及生长发育指标、并发症。结果观察组呕吐、腹胀消失时间以及达全胃肠喂养时间、恢复出生体重时间均短于对照组(P<0.05);观察组治疗后ALB、PA、TF以及体重增加量均高于对照组(P<0.05);观察组吸入性肺炎、低血糖、胃出血、胃潴留、便秘等并发症发生率低于对照组(P<0.05)。结论早期微量喂养联合肠外营养治疗对ICU早产儿有显著疗效,能加快胃肠功能恢复速度,改善营养状态,促进体重增长,且安全性较高。Objective To explore effect of early micro feeding combined with parenteral nutrition treatment on gastrointestinal function and growth and development level of premature infants in ICU.Methods The paper chose 60 premature infants in ICU of our hospital from January 2021 to January 2023,and divided them into two groups randomly.Control group was treated with parenteral nutrition,while observation group with early micro feeding combined with parenteral nutrition.Gastrointestinal function recovery indicators,nutritional and growth development indicators,and complications were compared between two groups.Results Disappearance time of vomiting and bloating,time to reach full gastrointestinal feeding and recovery of birth weight in observation group,was shorter than control group(P<0.05).After treatment,ALB,PA,TF and weight gain in observation group was higher than control group(P<0.05).Incidence of inhalation pneumonia,hypoglycemia,gastric bleeding,gastric retention,constipation and other complications in observation group was lower than control group(P<0.05).Conclusion Early micro feeding combined with parenteral nutrition therapy can achieve significant curative effect on premature infants in ICU,can accelerate recovery of gastrointestinal function,improve nutritional status and promote weight gain with high safety.
