摘 要:目的 研究皮肌炎患者外周血T淋巴细胞减少与视黄酸诱导基因-I(RIG-I)路径联系。方法 选取2020年8月—2023年8月收治的60例皮肌炎(皮肌炎组)及60例正常健康者(对照组)。检测分析2组外周血液中T淋巴细胞及RIG-I路径核因子-κB(NF-κB)、干扰素调节因子-3(IRF-3)、干扰素调节因子-7(IRF-7)蛋白表达水平及相关性。外周血T淋巴细胞与RIG-I路径相关蛋白相关性应用Pearson分析。结果 皮肌炎组CD4+T细胞、CD8+T细胞、CD4+/CD8+低于对照组,NF-κB蛋白、IRF-3蛋白、IRF-7蛋白、RIG-I蛋白高于对照组(P<0.01);活动期患者CD4+T细胞、CD8+T细胞、CD4+/CD8+低于非活动期患者,NF-κB蛋白、IRF-3蛋白、IRF-7蛋白、RIG-I蛋白高于非活动期患者(P<0.01);皮肌炎组、活动期患者CD4+T细胞、CD8+T细胞、CD4+/CD8+与NF-κB蛋白、IRF-3蛋白、IRF-7蛋白、RIG-I蛋白呈负相关(P<0.01);对照组、非活动期患者CD4+T细胞、CD8+T细胞、CD4+/CD8+与NF-κB蛋白、IRF-3蛋白、IRF-7蛋白、RIG-I蛋白无相关性(P>0.05)。结论 皮肌炎患者外周血CD4+T细胞、CD8+T细胞、CD4+/CD8+降低,RIG-I路径活性增强,二者均与病情活动情况有关,且外周血T淋巴细胞与RIG-I路径具有负调控作用,在皮肌炎发生与病情活动中可能起到相互作用。Objective To investigate the association between peripheral blood T lymphocytopenia and retinoic acid-induced gene-I(RIG-I)pathway in patients with dermatomyositis.Methods Sixty patients with dermatomyositis(dermatomyositis group)and 60 normal healthy subjects(control group)were selected from August 2020 to August 2023.The expression levels of T lymphocytes and RIG-I pathway nuclear factor-κB(NF-κB),interferon regulatory factor-3(IRF-3)and interferon regulatory factor-7(IRF-7)proteins in peripheral blood and their correlation in the two groups were detected and analyzed.The correlation between peripheral blood T lymphocytes and RIG-I pathway-related proteins was analyzed by Pearson analysis.Results CD4+T cells,CD8+T cells,and CD4+/CD8+were lower in dermatomyositis group than in control group,while NF-κB protein,IRF-3 protein,IRF-7 protein and RIG-I protein were higher than those in control group(P<0.01).CD4+T cells,CD8+T cells and CD4+/CD8+in patients in active stage were lower than those in patients in non-active stage,while NF-κB protein,IRF-3 protein,IRF-7 protein and RIG-I protein were higher than those in patients in non-active stage(P<0.01).CD4+T cells,CD8+T cells,and CD4+/CD8+were negatively correlated with NF-κB protein,IRF-3 protein,IRF-7 protein and RIG-I protein in dermatomyositis group and patients in active stage(P<0.01).There was no correlation of CD4+T cells,CD8+T cells,and CD4+/CD8+with NF-κB protein,IRF-3 protein,IRF-7 protein and RIG-I protein in control group and patients in non-active stage(P>0.05).Conclusion In patients with dermatomyositis,peripheral blood CD4+T cells,CD8+T cells,CD4+/CD8+are decreased,while RIG-I pathway activity is increased,both of which are related to disease activity.In addition,peripheral blood T lymphocytes and RIG-I pathway have negative regulatory effects,which may play an interaction in the occurrence and disease activity of dermatomyositis.
