鼻腔扩容术联合改良悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征患者的效果

摘　　要：目的:观察鼻腔扩容术联合改良悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者的效果。方法:回顾性分析2018年4月至2022年8月该院收治的100例OSAHS患者的临床资料,按治疗方法不同分为观察组与对照组各50例。对照组行改良悬雍垂腭咽成形术治疗,观察组在对照组基础上行鼻腔扩容术治疗,比较两组睡眠[匹兹堡睡眠质量指数(PSQI)、Epworth嗜睡量表(ESS)]评分、呼吸暂停低通气指数(AHI)、负性情绪[焦虑自评量表(SAS)、抑郁自评量表(SDS)]评分和睡眠呼吸暂停生命质量指数(SAQLI)评分。结果:术后1、6个月,观察组PSQI、ESS、SAS、SDS评分和AHI水平低于对照组,SAQLI评分高于对照组,差异均有统计学意义(P<0.05)。结论:鼻腔扩容术联合改良悬雍垂腭咽成形术治疗OSAHS患者可提高其SAQLI评分,降低其PSQI、ESS、SAS、SDS评分及AHI水平,效果优于单纯改良悬雍垂腭咽成形术治疗。Objective:To observe effects of nasal cavity expansion surgery combined with modified uvulopalatopharyngoplasty in treatment of patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods:The clinical data of 100 patients with OSAHS admitted to the hospital from April 2018 to August 2022 were retrospectively analyzed.According to different treatment methods,they were divided into observation group and control group,50 cases in each group.The control group was treated with modified uvulopalatopharyngoplasty,while the observation group was treated with nasal cavity expansion surgery on the basis of that of the control group.The sleep[Pittsburgh sleep quality index(PSQI),Epworth sleepiness scale(ESS)]score,the apnea hypopnea index(AHI),the negative emotions[self-rating anxiety scale(SAS),self-rating depression scale(SDS)]scores,and the sleep apnea quality of life index(SAQLI)score were compared between the two groups.Results:1 and 6 months after the surgery,the PSQI,ESS,SAS,SDS scores and the AHI levels of the observation group were lower than those of the control group,the SAQLI score was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusions:Nasal cavity expansion surgery combined with modified uvulopalatopharyngoplasty can improve the SAQLI scores and reduce the PSQI,ESS,SAS,SDS scores and the AHI levels in the patients with OSAHS.Moreover,it is superior to single modified uvulopalatopharyngoplasty.
