摘　　要：目的研究三维步态分析仪(3DGA)联合髋膝踝关节周围肌群肌力训练(PMST)用于全膝关节置换术(TKA)后康复的效果。方法随机对照研究。抽取2021年1月至2022年12月河南省中医院收治的TKA患者102例,按随机数字表法分为PMST组和3DGA组,每组51例。PMST组采用髋膝踝关节PMST,3DGA组在PMST组基础上使用3DGA干预。对比两组干预后时间-空间参数、运动学参数、膝关节力学参数及膝关节功能[美国特种外科医院膝关节评分(HSS)、美国膝关节协会评分法(KSS)]水平。结果干预12周后,3DGA组步速高于PMST组,步长长于PMST组,单侧患足支撑时间长于PMST组(P<0.05)。干预12周后,3DGA组脚触地后重心向全足转移时最大屈膝角度、脚离地时屈膝角度、对侧足尖离地时最大伸膝角度均大于PMST组(P均<0.05)。干预12周后,3DGA组矢状面第1次最大伸膝力矩、矢状面第2次最大伸膝力矩、矢状面最大屈膝力矩均小于PMST组(P均<0.05)。干预12周后,3DGA组HSS评分及KSS评分均高于PMST组(P均<0.05)。结论 3DGA联合髋膝踝关节PMST用于TKA后效果显著,能明显改善时间-空间参数、运动学参数及膝关节力学参数,恢复患者膝关节功能。Objective To study the application effect of 3-dimensional gait analyzer(3DGA)combined with hip,knee and ankle joint peripheral muscle strength training(PMST)in postoperative rehabilitation of total knee arthroplasty(TKA).Methods This was a randomized controlled trial.A total of 102 TKA patients treated in Henan Hospital of Traditional Chinese Medicine from January 2021 to December 2022 were selected,and they were divided into PMST group and 3DGA group by random number table method,with 51 cases in each group.The PMST group was treated by hip,knee and ankle joint PMST,while the 3DGA group was treated by 3DGA based on the treatment of the PMST group.The levels of temporal-spatial parameters,kinematic parameters,knee joint mechanical parameters,and knee joint function assessed by Hospital for Special Surgery knee score(HSS)and American Knee Society knee score(KSS)were compared between the two groups before and after 12 weeks of intervention.Results After 12 weeks of intervention,compared with the PMST group,the 3DGA group had higher gait speed,longer gait length,and longer unilateral foot support time(P<0.05).After 12 weeks of intervention,the maximum knee flexion angle when the center of gravity shifted to the full foot after the foot touched the ground,the knee flexion angle when the foot was off the ground,and the maximum knee extension angle when the contra-lateral tip of the foot was off the ground in the 3DGA group were larger than those in the PMST group(all P<0.05).After 12 weeks of intervention,the first maximum knee extension moment on the sagittal plane,the second maximum knee extension moment on the sagittal plane,and the maximum knee flexion moment on the sagittal plane in the 3DGA group were smaller than those in the PMST group(all P<0.05).After 12 weeks of intervention,the HSS and KSS scores in the 3DGA group were higher than those in the PMST group(all P<0.05).Conclusions 3DGA combined with hip,knee and ankle joint PMST after TKA can significantly improve the temporal-spatial parameters,kinematic pa
