摘 要:目的研究3.0T磁共振多参数成像(mp-MRI)结合动态增强扫描(DEC)诊断前列腺癌的意义。方法本研究为回顾性队列研究。抽取2021年12月至2022年11月南阳市中心医院收治的疑似前列腺癌患者64例,均行3.0T磁共振双参数成像(bp-MRI)诊断和3.0T mp-MRI结合DEC诊断,以病理检查结果为"金标准",统计3.0T bp-MRI和3.0T mp-MRI结合DEC诊断的诊断结果和诊断效能,分析预测值。结果 64例疑似前列腺癌患者,经病理检查确诊阳性35例,阴性29例;3.0T bp-MRI诊断出真阳性27例,真阴性24例;3.0T mp-MRI结合DEC诊断出真阳性34例,真阴性28例。3.0T mp-MRI结合DEC诊断灵敏度(97.14%,34/35);准确度(96.88%,62/64)高于3.0T bp-MRI诊断(77.14%,27/35;81.25%,52/64),漏诊率(2.86%,1/35)低于3.0T bp-MRI诊断(22.86%,8/35),P<0.05;3.0T mp-MRI结合DEC诊断的阴性预测值(96.55%,28/29)高于3.0T bp-MRI诊断(75.00%,24/32),P<0.05。结论 3.0T mp-MRI结合DEC诊断前列腺癌可提升诊断灵敏度和准确度,降低漏诊率,且预测值良好。Objective To investigate the significance of 3.0T multi-parameter magnetic resonance imaging(mp-MRI)and dynamic contrast enhanced scan(DEC)in the diagnosis of prostate cancer.Methods This study was a retrospective cohort study.A total of 64 patients with suspected prostate cancer treated in Nanyang Central Hospital from December 2021 to November 2022 were selected.All patients underwent 3.0T bi-parampetric magnetic resonance imaging(bp-MRI)and 3.0T mp-MRI combined with DEC for diagnosis.With pathological examination results as the gold standard,the diagnostic results and diagnostic efficacy of 3.0T bp-MRI and 3.0T mp-MRI combined with DEC were analyzed,and the predictive value was analyzed.Results Among the 64 patients with suspected prostate cancer,35 cases were positive,and 29 cases were negative according to pathological examination.3.0T bp-MRI identified 27 cases of true positive and 24 cases of true negative;3.0T mp-MRI combined with DEC identified 34 cases of true positive and 28 cases of true negative.The sensitivity and accuracy of 3.0T mp-MRI combined with DEC(97.14%,34/35;96.88%,62/64)were higher than those of 3.0T bp-MRI(77.14%,27/35;81.25%,52/64),P<0.05;the missed diagnosis rate of 3.0T mp-MRI combined with DEC(2.86%,1/35)was lower than that of 3.0T bp-MRI(22.86%,8/35),P<0.05.The negative predictive value of 3.0T mp-MRI combined with DEC in the diagnosis of prostate cancer was 96.55%(28/29),which was higher than 75.00%(24/32)of 3.0T bp-MRI(P<0.05).Conclusions 3.0T mp-MRI combined with DEC in the diagnosis of prostate cancer can improve the diagnostic sensitivity and accuracy,and reduce the rate of missed diagnosis,with good predictive value.
