血清BNP、hs-CRP、cTnI、UA联合检测在心力衰竭诊断中的应用价值

摘　　要：目的分析血清B型钠尿肽(BNP)、超敏C反应蛋白(hs-CRP)、肌钙蛋白I(cTnI)、尿酸联合检测在心力衰竭诊断中的应用价值。方法抽取2022年3月至2023年3月濮阳市安阳地区医院收治的心力衰竭患者37例为观察组,并抽取同期健康体检者37例为对照组。比较两组血清BNP、hs-CRP、cTnI、尿酸水平。比较观察组不同心功能分级患者血清BNP、hs-CRP、cTnI、尿酸水平。分析血清BNP、hs-CRP、cTnI、尿酸水平和心力衰竭患者心功能分级的相关性。比较BNP、hs-CRP、cTnI、尿酸单独检测和联合检测的阳性率。结果观察组血清BNP、hs-CRP、cTnI、尿酸水平低于对照组(P<0.05)。观察组心功能Ⅳ级患者血清BNP、hs-CRP、cTnI、尿酸水平高于Ⅰ、Ⅱ、Ⅲ级患者(P<0.05)。Pearson分析结果显示,BNP(r=0.671)、hs-CRP(r=0.618)、cTnI(r=0.704)、尿酸(r=0.709)和心功能分级呈正相关(P<0.05)。BNP、hs-CRP、cTnI、尿酸阳性率分别为67.57%(25/37)、45.95%(17/37)、72.97%(27/37)、72.97%(27/37),BNP、hs-CRP、cTnI、尿酸联合检测阳性率为91.89%(34/37),高于BNP、hs-CRP、cTnI、尿酸单独诊断(P<0.05)。结论心力衰竭患者血清BNP、hs-CRP、cTnI、尿酸表达程度较高,且心功能越差其表达水平越高,联合检测可提高阳性率,为临床诊断提供参考。Objective To analyze the value of combined detection of serum B-type natriuretic peptide(BNP),hypersensitive C-reactive protein(hs-CRP),cardiac troponin I(cTnI)and uric acid in the diagnosis of heart failure.Methods A total of 37 patients with heart failure treated in Anyang District Hospital of Puyang City from March 2022 to March 2023 were selected as the observation group,and 37 healthy people who underwent physical examination during the same period were selected as the control group.The levels of serum BNP,hs-CRP,cTnI and uric acid were compared between the two groups.The levels of serum BNP,hs-CRP,cTnI and uric acid in patients with different cardiac function classifications were compared in the observation group.The correlations of levels of serum BNP,hs-CRP,cTnI and uric acid with cardiac function classification in patients with heart failure were analyzed.The positive rates of BNP,hs-CRP,cTnI,UA and combination of them in the diagnosis of heart failure were compared.Results The levels of serum BNP,hs-CRP,cTnI and uric acid in the observation group were lower than those in the control group(P<0.05).The levels of serum BNP,hs-CRP,cTnI and uric acid in patients with gradeⅣcardiac function were higher than those in patients with gradeⅠ,ⅡandⅢcardiac function in the observation group(P<0.05).Results of pearson analysis showed that BNP(r=0.671),hs-CRP(r=0.618),cTnI(r=0.704)and uric acid(r=0.709)were positively correlated with cardiac function classification(P<0.05).The positive rates of BNP,hs-CRP,cTnI and uric acid in the diagnosis of heart failure were 67.57%(25/37),45.95%(17/37),72.97%(27/37)and 72.97%(27/37),respectively.The positive rate of combined detection of BNP,hs-CRP,cTnI and uric acid was 91.89%(34/37),which was higher than that of the four single factors(P<0.05).Conclusions The levels of serum BNP,hs-CRP,cTnI and uric acid in patients with heart failure are higher,and the heart function get worse with the increase in the levels of serum BNP,hs-CRP,cTnI and uric acid.Combined detection of th
