摘 要:目的探讨产前超声联合孕妇血锌、碱性磷酸酶(ALP)、乔治综合征危象基因8(DGCR8)检测对胎儿先天性心脏病的诊断价值。方法抽取2021年4月至2023年1月于河南大学第一附属医院产检的92例孕妇(单胎妊娠),其中40例诊断为胎儿先天性心脏病的孕妇设为研究组,52例胎儿无先天性心脏病的孕妇设为对照组。两组均给予产前超声检查以及血清标志物(血锌、DGCR8、ALP)检测,采用受试者工作特征曲线下面积(AUC)分析产前超声、血清标志物单项及联合检测的诊断效能。结果研究组胎儿心轴角度大于对照组,差异有统计学意义(P<0.05)。研究组血锌、ALP及DGCR8水平低于对照组,差异有统计学意义(P<0.05)。产前超声、血锌、ALP、DGCR8四者联合,诊断胎儿先天性心脏病的灵敏度(98.6%)、AUC(0.951)和准确率(89.0%)高于四者单项诊断。结论产前超声联合血锌、ALP及DGCR8检测对胎儿先天性心脏病的鉴别诊断效能较高,具有较好的应用价值。Objective To investigate the diagnostic value of prenatal ultrasound combined with blood zinc,alkaline phosphatase(ALP),and DiGeorge syndrome critical region gene 8(CDGCR8)for fetal congenital heart disease.Methods A total of 92 pregnant women(pregnant with a single fetus)who underwent prenatal examination in the First Affiliated Hospital of Henan University from April 2021 to January 2023 were selected,of which 40 pregnant women diagnosed with fetal congenital heart disease were set as the study group,and 52 pregnant women without fetal congenital heart disease were set as the control group.Prenatal ultrasonography and serum markers,including blood zinc,DGCR8 and ALP,were detected in both groups.The area under the receiver operating characteristic curve(AUC)was used to analyze the diagnostic efficacy of prenatal ultrasound,serum markers and combined detection of them.Results The fetal axial angle in the study group was larger than that in the control group(P<0.05).The levels of blood zinc,ALP and DGCR8 in the study group were lower than those in the control group,and the differences were significant(P<0.05).The sensitivity(98.6%),AUC(0.951)and accuracy(89.0%)of combination of prenatal ultrasound,blood zinc,ALP and DGCR8 in the diagnosis of fetal congenital heart disease were higher than those of the four diagnostic methods alone.Conclusions Prenatal ultrasound combined with blood zinc,ALP and DGCR8 detection has great efficacy in the differential diagnosis of fetal congenital heart disease,and it has good application value.
