摘 要:目的分析生物型全髋关节置换术中应用带线锚钉修复髋关节后外侧结构对股骨颈骨折患者术后髋关节Harris评分的影响。方法抽取2019年10月至2022年10月南阳市中心医院诊治的股骨颈骨折患者作为研究对象。按手术方法将166例患者分为修复组(83例)和未修复组(83例)。两组均行生物型全髋关节置换术,修复组在行生物型全髋关节置换术中应用带线锚钉修复髋关节后外侧结构。观察两组患者术中以及术后3个月脱位情况、术前以及术后3个月的Harris评分和两组患者术后不良反应发生情况。结果修复组术中髋关节屈曲100°时脱位率(3.61%,3/83)低于未修复组(12.05%,10/83),P<0.05。术后3个月,修复组脱位率(0)低于未修复组(4.82%,4/83),P<0.05;两组患者术后的Harris评分均较术前升高,术后3个月修复组Harris评分高于未修复组(P<0.05)。未修复组患者出现1例假体感染,其余患者未出现严重并发症及不良事件。结论带线锚钉修复髋关节后外侧结构对股骨颈骨折患者行生物型全髋关节置换术后髋关节具有良好的修复作用,能够提高Harris评分,减少假体脱位。Objective To analyze the influence of repairing posterolateral structure of hip joint with wire anchor on the postoperation Harris score of hip joint in biological total hip arthroplasty for femoral neck fractures.Methods A total of 166 cases of femoral neck fractures diagnosed and treated in Nanyang Central Hospital from October 2019 to October 2022 were selected as the study subjects.They were divided into repair group(83 cases)and non-repair group(83 cases)by surgical method.Biological total hip arthroplasty was performed in both groups.In the repair group,the posterolateral structure of hip joint was repaired with wire anchor.The dislocation during operation and 3 months after operation,preoperative and 3-months postoperative Harris score,and adverse reactions of the two groups were observed.Results The dislocation rate during hip joint flexion at 100°(3.61%,3/83)in the repair group was lower than that in the non-repair group(12.05%,10/83),P<0.05.Three months after operation,the dislocation rate in the repair group(0.00%,0/83)was lower than that in the non-repair group(4.82%,4/83),P<0.05.Three months after operation,the Harris scores in the two groups were higher than preoperative scores,and the Harris score in the repair group was significantly higher than that in the non-repair group(P<0.05).There was 1 case of prosthesis infection in the non-repair group;no serious complication or adverse events occurred in the remaining patients.Conclusions Repairing posterolateral structure of the hip joint with wire anchor has a good repair effect on the hip joint of patients after total hip arthroplasty for femoral neck fractures,which can improve Harris score,reduce prosthesis dislocation.
