血清Lp-PLA2、H-FABP结合UA对急性心肌梗死急诊PCI术后心力衰竭的预测分析

摘　　要：目的分析血清脂蛋白相关磷脂酶A2(Lp-PLA2)、心肌型脂肪酸结合蛋白(H-FABP)结合尿酸(UA)对急性心肌梗死(AMI)急诊经皮冠状动脉介入术(PCI)后心力衰竭(HF)的预测价值。方法选择2020年2月—2023年2月本院收治的行急诊PCI术的AMI患者312例作为研究对象,用Logistic回归模型分析术后6个月HF的影响因素;绘制受试者工作特征曲线(ROC)分析血清Lp-PLA2、H-FABP结合UA对HF的预测价值。结果AMI急诊PCI术后6个月HF发生率为24.23%;Logistic回归模型分析显示,年龄、冠脉Gensini积分、发病至PCI时间、院外用药依从及血清Lp-PLA2、H-FABP、UA是AMI急诊PCI术后HF的影响因素(P<0.05);ROC曲线显示,血清Lp-PLA2、H-FABP结合UA预测AMI急诊PCI术后HF的灵敏度及曲线下面积(AUC)均高于单独预测(P<0.05)。结论血清Lp-PLA2、H-FABP、UA是AMI急诊PCI术后HF的影响因素,三者结合的预测效能高于单独预测。Objective To analyze the value of serum lipoprotein-associated phospholipase A2(Lp-PLA2),heart-type fatty acid binding protein(H-FABP)combined with uric acid(UA)in predicting heart failure after emergency percutaneous coronary intervention(PCI)due to acute myocardial infarction(AMI).Methods A total of 312 patients with AMI who underwent emergency PCI from February 2020 to February 2023 were selected as study subjects,and the influencing factors of heart failure were analyzed by using Logistic regression model.Receiver operating characteristic curve(ROC)was drawn to analyze the value of serum Lp-PLA2,H-FABP combined with UA in predicting heart failure after emergency PCI due to AMI.Results The incidence of heart failure within 6 months after emergency PCI due to AMI was 24.23%.Logistic regression model analysis showed that age,coronary Gensini score,time from onset to PCI,medication compliance outside hospital,and serum levels of Lp-PLA2,H-FABP,and UA were influencing factors of heart failure after emergency PCI due to AMI(P<0.05).ROC curve showed that serum Lp-PLA2,H-FABP combined with UA had higher sensitivity and area under the curve(AUC)in predicting heart failure after AMI emergency PCI than that predicted alone(P<0.05).Conclusions Serum Lp-PLA2,H-FABP,and UA are influencing factors of heart failure after AMI emergency PCI,and the predictive efficacy of the combination of the three is higher than that of single prediction.
