降钙素原、C反应蛋白与血浆游离DNA联合检测在烧伤患者脓毒症早期诊断中的价值探究

摘　　要：目的探究降钙素原(PCT)、C反应蛋白(CRP)与血浆游离DNA联合检测在烧伤患者脓毒症早期诊断中的价值。方法回顾性分析2021年1月—2022年1月本院收治的60例烧伤患者的临床资料,根据是否出现脓毒症,将出现脓毒症的烧伤患者纳入脓毒症组(n=27),未出现脓毒症的患者纳入对照组(n=33),比较两组的临床特征及PCT、CRP、血浆游离DNA水平差异,并分析PCT、CRP、血浆游离DNA联合检测烧伤患者脓毒症的诊断效能。结果脓毒症组的PCT、CRP与游离DNA均高于对照组(P<0.05);PCT、CRP、血浆游离DNA单一检测及联合检测的敏感度总体比较,差异有统计学意义(P<0.05);其中三者联合检测的敏感度高于PCT、CRP、血浆游离DNA单一检测(P<0.05);PCT、CRP、血浆游离DNA单一检测及联合检测的特异度和准确度总体比较,差异无统计学意义(P>0.05)。结论PCT、CRP和血浆游离DNA联合检测在烧伤患者脓毒症早期诊断中具备较高的诊断效能,具有积极意义。Objective To explore the value of combined detection of procalcitonin(PCT),C-reactive protein(CRP),and plasma cell-free DNA in the early diagnosis of sepsis in burn patients.Methods The clinical data of 60 burn patients who were admitted to our hospital from January 2021 to January 2022 were retrospectively analyzed.According to the presence or absence of sepsis,burn patients with sepsis were included in sepsis group(n=27),and patients without sepsis were included in control group(n=33).The differences in clinical characteristics,PCT,CRP,and plasma free DNA levels between the two groups were compared,and the diagnostic efficacy of combined detection of PCT,CRP,and plasma free DNA in burn patients with sepsis was analyzed.Results The sepsis group had significantly higher levels of PCT,CRP,and cell-free DNA than the control group(P<0.05).There was a statistically significant difference in the sensitivity of PCT,CRP,and plasma cell-free DNA single detection and combined detection(P<0.05).The sensitivity of the combined detection of the three indicators was higher than that of PCT,CRP and plasma cell-free DNA(P<0.05).There was no significant difference in the specificity and accuracy of PCT,CRP,and plasma cell-free DNA single detection and combined detection(P>0.05).Conclusions The combined detection of PCT,CRP,and plasma cell-free DNA presents high diagnostic efficiency in the early diagnosis of sepsis in burn patients and has positive significance.
