亚甲蓝注射联合痔瘘祛毒熏洗剂坐浴用于环状混合痔术后的疗效观察

摘　　要：目的:探究亚甲蓝注射联合痔瘘祛毒熏洗剂坐浴用于环状混合痔术后的疗效。方法:选取2021年1月至2022年6月,本院手术治疗的182例环状混合痔患者,随机分为治疗组和对照组,各91例。2组患者均行混合痔外剥内扎术治疗,对照组术后给予芪蓉润肠口服液口服联合常规抗生素静脉滴注。在此基础上,治疗组患者局部注射亚甲蓝混合液联合中药痔瘘祛毒熏洗剂坐浴,比较2组临床疗效及并发症情况。结果:治疗组总有效率显著高于对照组(89.01%VS 72.53%,P<0.05)。治疗结束后,治疗组疼痛、水肿、出血、渗液、肉芽组织及创面面积评分均显著低于对照组(P<0.05)。治疗组患者血清白介素(IL)-2、IL-6以及肿瘤坏死因子(TNF-α)水平均显著低于对照组(P<0.05)。上皮开始覆盖时间和创面愈合时间均短于对照组(P<0.05)。治疗组复发率显著低于对照组(4.40%VS 16.48%,P<0.05)。结论:环状混合痔术后采用亚甲蓝局部注射联合痔瘘祛毒熏洗剂坐浴可提高临床疗效,加快创面愈合时间,减少并发症,能够有效改善患者预后,具有临床应用价值。Objective To explore the clinical efficacy of methylene blue injection combined with Zhilou Qudu Formula for Fumigation and Washing sitz bath in postoperative annular mixed hemorrhoid.Meth-ods From January 2021 to June 2022,total 182 patients treated with Milligan-Morgan hemorrhoidectomy(MMH)for mixed hemorrhoids in our hospital were randomly divided into the treatment group and the control group,91 cases in each group.The control group was given Qirong Runchang Oral Liquid com-bined with conventional antibiotics intravenously.On this basis,patients in the treatment group were injec-ted with methylene blue mixture combined with Zhilou Qudu Formula for Fumigation and Washing sitz bath,and the clinical efficacy and complications of the two groups were compared.Results The total ef-fective rate of the treatment group was significantly higher than that of the control group(89.01%VS 72.53%,P<0.05).After treatment,the scores of pain,edema,bleeding,exudation,granulation tissue and wound area in the treatment group were significantly lower than those in the control group(P<0.05).Serum levels of IL-2,IL-6 and TNF-αin the treatment group were significantly lower than those in the control group(P<0.05).The epithelial coverage time and wound healing time were shorter than those of the control group(P<0.05).The recurrence rate in the treatment group was significantly lower than that in the control group(4.40%VS 16.48%,P<0.05).Conclusion Local injection of methylene blue combined with Zhilou Qudu Formula for Fumigation and Washing sitz bath in postoperative annular mixed hemorrhoid can improve clinical efficacy,accelerate wound healing time,reduce complications,and effec-tively improve patient prognosis,which has clinical application value.
