摘 要:目的:基于炎症因子表达观察柴胡龙骨牡蛎汤加味治疗癫痫的作用机制。方法:40只SD雄性大鼠随机分为正常对照组、模型组、中药组、西药组。采用戊四氮(PTZ)腹腔注射造模结束后,每组均予治疗4周,模型组和正常对照组每日给予0.9%氯化钠灌胃,西药组给予丙戊酸钠缓释片灌胃,中药组给予汤剂柴胡龙骨牡蛎汤加味灌胃。造模开始及治疗期间,对各组大鼠行为学进行观察。治疗28天后,检测各组大鼠血清中癫痫相关炎症因子肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-10(IL-10)的变化,HE染色,观察海马结构的病理学变化情况。结果:(1)各组大鼠行为学变化:西药组及中药组大鼠癫痫发作的次数和级别都有所减少,体重有增加趋势;中药组发作分级及平均癫痫发作时间降低明显。(2)各组大鼠海马结构变化:与模型组相比,西药组海马组织细胞损伤程度减轻,神经元少量变性,偶见胶质细胞增生;中药组海马组织细胞损伤程度较轻,神经元丢失及变性不明显。(3)炎症因子检测:中药组和西药组的IL-6和TNF-α与模型组相比,表达明显降低,IL-10表达明显升高,中药组与西药组比较,差异无统计学意义(P>0.05)。结论:柴胡龙骨牡蛎汤加味可能通过下调癫痫大鼠血清炎症因子IL-6、TNF-α的表达,升高IL-10表达,从而减少癫痫大鼠海马神经元损伤,保护海马神经元细胞,起到控制癫痫发作次数和发作频率,缩短发作时间的作用。[Objective]This study aimed to observe the mechanism of action of Chaihu Longgu Muli Tang plus modified treatment on epilepsy based on the expression of inflammatory factors.[Methods]Forty male SD rats were randomly divided into the normal control group,model group,traditional Chinese medicine(TCM)group,and Western medicine group.After modeling using pentylenetetrazol(PTZ)intraperitoneal injection,each group received treatment for 4 weeks.The model group and normal control group were orally administered 0.9%saline daily.The Western medicine group was orally administered sustained-release sodium valproate tablets,and the TCM group was orally administered modified Chaihu Longgu Muli Tang.Behavioral observations were conducted on the rats in each group during modeling and treatment.After 28 days of treatment,changes in the levels of epilepsy-related inflammatory factors tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),and interleukin-10(IL-10)were measured in the serum of the rats in each group.HE staining was performed to observe the histopathological changes in the hippocampus.[Results]①Behavioral changes in rats:The number and severity of seizures in the Western medicine group and TCM group were reduced,and there was a trend of increased body weight.The TCM group showed a significant reduction in seizure grading and average seizure duration.②Hippocampal structural changes in rats:Compared with the model group,the Western medicine group showed less damage to hippocampal tissue cells,with a small number of neurons showing degeneration and occasional glial cell proliferation.The TCM group showed milder damage to hippocampal tissue cells,with no significant loss or degeneration of neurons.③Inflammatory factor analysis:Compared with the model group,the expression of IL-6 and TNF-αwassignificantly decreased,while the expression of IL-10 was significantly increased in both of the TCM group and Western medicine group.There was no significant difference between the TCM group and Western medicine group(P>0.05
