温阳益气化饮通络法治疗慢性心力衰竭临床疗效及对心功能、血浆NT-proBNP的影响

摘　　要：目的:研究温阳益气化饮通络法治疗慢性心力衰竭临床疗效及对心功能、血浆NT-proBNP的影响。方法:选取2022年1月~2023年2月于我院心内科就诊的CHF患者180例,随机数字法分为对照组和试验组,每组90例。对照组采用常规抗CHF治疗,试验组采用温阳益气化饮通络法治疗;比较两组临床疗效、心排出量(CO)、左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、脑利钠肽(BNP)及N端脑利钠肽前体(NT-proBNP)的水平。结果:两组疗效等级差异有统计学意义,试验组总有效率为95.56%,显著高于对照组的81.11%(P<0.05);两组治疗后心悸气短、颈部青筋暴露、下肢浮肿等主症积分,次症积分及总分较治疗前显著下降,其中试验组治疗后上述积分显著低于对照组(P<0.05);两组治疗后CO、LVEF较治疗前显著上升,其中试验组治疗后CO、LVEF水平显著高于对照组(P<0.05),但两组治疗前后LVEDD及LVESD比较差异无统计学意义(P>0.05);两组治疗后NT-proBNP及BNP较治疗前显著下降,且试验组治疗后NT-proBNP及BNP显著低于对照组(P<0.05)。结论:温阳益气化饮通络法可有效治疗慢性心力衰竭,且有效改善心功能,降低血浆NT-proBNP。Objective:To study the clinical effect of Wenyang Yiqi Huayin Tongluo Prescription in the treatment of chronic heart failure,and the influence on cardiac function and plasma N-terminal prohormone of brain natriuretic peptide(NT-proBNP).Methods:A total of 360 patients with CHF admitted to the hospital from May 2019 to May 2021 were selected,and divided into control group and experimental group by random number table method with 180 cases in each group.Patients in the control group were given conventional anti-CHF therapy,and patients in the experimental group were treated with Wenyang Yiqi Huayin Tongluo Prescription.Clinical effect,cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),brain natriuretic peptide(BNP)and NT-proBNP levels were compared between the two groups.Results:The total response rate in the experimental group was significantly higher than that in the control group(96.11%vs 82.22%,P<0.05).After treatment,the scores for main symptoms and secondary symptoms,and the total score were significantly decreased in the two groups,and the experimental group had significantly lower scores than the control group(P<0.05).After treatment,CO and LVEF of the two groups were significantly increased,and the experimental group had significantly higher CO and LVEF than the control group(P<0.05).There was no significant difference in LVEDD or LVESD between the two groups before and after treatment(P>0.05).After treatment,the levels of NT-proBNP and BNP were significantly decreased in the two groups,and significantly lower in the experimental group than in the control group(P<0.05).Conclusion:Wenyang Yiqi Huayin Tongluo Prescription is effective in the treatment of chronic heart failure.It can effectively improve cardiac function,and lower plasma NT-proBNP level in the patients.
