嗜血支原体感染患猫继发埃文斯综合征的病例报告

摘　　要：本文报道了1例加州型嗜血支原体感染患猫疑似继发埃文斯综合征的病例。患猫因精神沉郁、皮肤散在淤血斑和血便来本院就诊,体格检查发现患猫全身皮肤和黏膜苍白伴自发出血,血常规检查提示贫血和严重血小板减少,血清生化检查显示肌酸激酶和天冬氨酸转氨酶显著升高,库姆斯试验结果呈阳性,PCR检查结果显示加州型嗜血支原体阳性,超声检查发现胆囊壁水肿、脾肿大和多个淋巴结增大,诊断为加州型嗜血支原体感染并继发埃文斯综合征。给予患猫糖皮质激素、多西环素和支持性治疗后好转,完全停药后未再复发。本病例是国内外首次报道加州型嗜血支原体感染猫后可导致血小板减少,继发埃文斯综合征,这一新发现提示临床兽医师在血小板减少患猫诊疗中应将嗜血支原体感染纳入鉴别诊断列表。This report presents a case of suspected secondary Evans syndrome in a cat infected with the Candidatus Mycoplasma haemominutum.The cat was presented to the hospital with symptoms of depression,scattered petechiae on the skin,and hematochezia.Physical examination revealed generalized pallor of the skin and mucous membranes accompanied by spontaneous bleeding.Hematological examination indicated anemia and severe thrombocytopenia.Serum biochemical analysis showed significantly elevated creatine kinase and aspartate aminotransferase.The Coombs test was positive,and PCR examination confirmed the presence of the Candidatus M.haemominutum.Ultrasonography revealed gallbladder wall edema,splenomegaly,and enlargement of multiple lymph nodes,leading to a diagnosis of Candidatus M.haemominutum infection with secondary Evans syndrome.The cat showed improvement after treatment with corticosteroids,doxycycline,and supportive care,with no recurrence after complete cessation of medication.This case is the first reported instance globally of a cat infected with the Candidatus M.haemominutum leading to thrombocytopenia and secondary Evans syndrome.This novel finding suggests that clinical veterinarians should consider hemotrophic mycoplasma infection in the differential diagnosis when dealing with thrombocytopenic cats.
