基于浊邪理论探讨仙方活命饮加减方修复不育症患者精子DNA完整结构的机制研究

摘　　要：目的探讨基于浊邪理论的仙方活命饮加减方修复不育症患者精子DNA完整结构(DFI)的作用机制。方法选取2021年9月至2022年12月南昌市生殖医院生殖男科门诊收治的符合男性不育症伴有精子DFI偏高的94例患者作为研究对象,采用掷硬币法分为观察组(51例)和对照组(43例),观察组采用仙方活命饮加减中药制成膏方,对照组采用左卡尼汀口服溶液治疗,治疗3个月,观察治疗后两组患者的精子DFI水平、前向运动精子活率、总活率以及中医症状积分。结果观察组治疗后DFI指标低于对照组,差异有统计学意义(P<0.05)。观察组前向运动精子活率、精子总活率高于对照组,中医症状积分低于对照组,差异有统计学意义(P<0.05)。结论仙方活命饮加减方能有效提高不育症患者前向运动精子活率、总活率,修复精子DFI,还能有效缓解中医症状,为临床治疗男性不育症提供新的治疗思路。Objective To explore the mechanism of Xianfang Huoming Decoction in repairing the DNA fragmentation index(DFI)in infertile patients based on the theory of turbid pathogen.Methods A total of 94 male infertility patients with high sperm DFI admitted to the Reproductive Andrology Clinic of Nanchang Reproductive Hospital from September 2021 to December 2022 were selected as the research objects.They were divided into observation group(51 cases)and control group(43 cases)by coin toss method.The observation group was treated with Xianfang Huoming Decoction plus or minus traditional Chinese medicine paste,the control group was treated with Levocarnitine Oral Solution,two groups were treated for 3 months,and the sperm DFI level,forward motile sperm viability,total sperm viability and traditional Chinese medicine symptom score of the two groups were observed after treatment.Results The DFI index of the observation group was lower than that of the control group after treatment,and the difference was statistically significant(P<0.05).The forward motility sperm viability and total sperm viability in the observation group were higher than those in the control group,and the traditional Chinese medicine symptom score was lower than that in the control group,with statistical significances(P<0.05).Conclusion Xianfang Huoming Decoction can effectively improve the forward motile sperm viability,total viability,repair sperm DFI,relieve the symptoms of traditional Chinese medicine,and provide a new therapeutic idea for the clinical treatment of male infertility.
