摘　　要：黄素化未破裂综合征(LUFS)的发病机制尚未明确,主要包括激素失调、免疫因子、疾病因素和血流变化等。激素失调指卵泡刺激素(FSH)、促黄体生成素(LH)水平及比例失调影响卵泡的成熟和排出;免疫系统异常活化干扰卵泡的发育和排卵;子宫内膜异位症等疾病影响LH/人绒毛膜促性腺激素受体(LHCGR)表达、环氧化酶-2(COX-2)表达与卵巢及子宫动脉血流量的减少,也参与LUFS的发生。目前,LUFS的诊断主要通过超声检查卵泡动态变化,并结合激素和基础体温变化判断,但早期症状不典型,需要连续监测以降低漏诊风险。中医采用针灸、中药等方法调节体内脏腑功能;西医采用药物促进卵泡成熟破裂;中西医结合治疗具有协同优势。但是LUFS的发病机制有待进一步阐明,诊疗水平也需提高敏感度和特异度。本文综述LUFS的发病机制及诊疗进展,建立LUFS预测模型将有助于其早期识别和干预。The pathogenesis of luteinized unruptured follicle syndrome(LUFS)has not been clarified.It mainly includes hormone imbalance,immune factors,disease factors and blood flow changes.Hormone imbalance refers to the imbalance of follicle stimulating hormone(FSH)and luteinizing hormone(LH)levels and proportions affecting follicle maturation and excretion.Abnormal activation of the immune system interferes with follicle development and ovulation;Endometriosis and other diseases affect LH/luteinizing hormone/choriogonadotropin receptor(LHCGR)expression,cyclooxygenase-2(COX-2)expression and decreased blood flow in ovarian and uterine arteries,and also participate in the occurrence of LUFS.At present,the diagnosis of LUFS is mainly determined by ultrasonography of follicular dynamic changes combined with hormonal and basal body temperature changes,but the early symptoms are not typical and continuous monitoring is required to reduce the risk of missed diagnosis.Traditional Chinese medicine adopts acupuncture,traditional Chinese medicine and other methods to regulate the viscera function in the body.Western medicine uses drugs to promote follicle maturation rupture;The treatment of integrated Chinese and western medicine has synergistic advantages.However,the pathogenesis of LUFS needs to be further elucidated,and the level of diagnosis and treatment needs to improve sensitivity and specificity.This article reviews the pathogenesis,diagnosis and treatment progress of LUFS,and the establishment of a prediction model for LUFS will be helpful for its early recognition and intervention.
