摘 要:目的探讨彩色多普勒超声血流动力学参数对孕晚期脑损伤胎儿的预测价值。方法选取60例行彩色多普勒超声检查并住院分娩的足月妊娠孕妇,其中32例经临床诊断为孕晚期脑损伤胎儿的孕妇作为研究组,28例正常妊娠孕妇作为对照组。对比两组检测结果。结果研究组胎儿脐动脉(UA)血流动力学参数均高于对照组(P<0.05);研究组大脑中动脉(MCA)血流动力学参数均低于对照组(P<0.05);根据彩色多普勒超声血流动力学参数检测结果显示,三者联合诊断价值最高,阳性31例,阴性29例,敏感度为93.75%,特异度为96.43%,阳性预测值为96.77%,阴性预测值为93.10%。结论孕晚期胎儿脑损伤可引起UA、MCA血流动力学参数的变化,且三者联合检测具有较高的敏感度,可提高临床诊断效率。Objective To investigate the value of color doppler ultrasound hemodynamic parameters in predicting fetal brain injury in late pregnancy.Methods 60 full-term pregnant women who underwent color doppler ultrasonography and were hospitalized for delivery were selected,among which 32 pregnant women with clinically diagnosed fetal brain injury in late pregnancy were selected as study group and 28 pregnant women with normal pregnancy were selected as control group.The results of the two groups were compared.Results The hemodynamic parameters of fetal umbilical artery(UA)in study group were higher than those in control group(P<0.05).The hemodynamic parameters of middle cerebral artery(MCA)in study group were lower than those in control group(P<o.05).According to the results of color doppler ultrasound hemodynamic parameters,the combined diagnostic value of the three was the highest,with 31 positive cases and 29 negative cases,sensitivity was 93.75%,specificity was 96.43%,positive predictive value was 96.77%,and negative predictive value was 93.10%.Conclusion Fetal brain injury in late pregnancy can cause changes in UA and MCA hemodynamic parameters,and the combined detection of the three has high sensitivity,which can improve the clinical diagnosis efficiency.
