摘　　要：目的探讨CA125、PRL、β-HCG对先兆流产的诊断价值。方法选取本院收治的50例正常妊娠孕妇为对照组、50例先兆流产孕妇为研究组,比较两组孕5周、孕9周的血清催乳素(PRL)、人绒毛膜促性腺激素(β-HCG)、糖类抗原125(CA125)水平,探究CA125、PRL、β-HCG对先兆流产的诊断价值。结果孕5周、孕9周,两组PRL、CA125、β-HCG逐渐递增,研究组CA125水平高于对照组,β-HCG、PRL水平低于对照组(P<0.05)。ROC曲线分析得知,PRL、CA125、β-HCG和联合检测的曲线下面积分别为0.899、0.948、0.938和0.989。联合检测的敏感度和特异度分别为100%和98.00%,显著高于单个指标检测。Kappa检验得知,联合检测检验妊娠结局的系数为0.980(P<0.05)。结论CA125、PRL、β-HCG联合检测对先兆流产诊断价值较高,值得临床推广。Objective To explore the diagnostic value of CA125,PRL andβ-HCG in threatened abortion.Methods 50 normal pregnant women admitted to our hospital were selected as the control group and 50 threatened miscarriage women as the research group.The serum prolactin(PRL),human chorionic gonadotropin(β-HCG)and carbohydrate antigen 125(CA125)levels of the two groups at 5 and 9 weeks of pregnancy were compared,exploring the diagnostic value of CA125,PRL andβ-HCG in threatened abortion.Results At 5 weeks and 9 weeks of pregnancy,both groups of PRL,CA125,β-HCG gradually increased,and the CA125 level in the study group was higher than that in the control group,The levels ofβ-HCG and PRL were lower than those of the control group(P<0.05).The ROC curve analysis showed that the areas under the curves for PRL,CA125,β-HCG and joint detection were 0.899,0.948,0.938,and 0.989,respectively.The sensitivity and specificity of joint detection were 100%and 98.00%,respectively,significantly higher than those of single indicator detection.The Kappa test showed that the coefficient of combined testing for pregnancy outcome was 0.980(P<0.05).Conclusion The combined detection of CA125,PRL andβ-HCG has high diagnostic value for threatened abortion and is worthy of clinical promotion.
