摘 要:目的探讨慢性心力衰竭(CHF)患者血清促红细胞生成素2相关因子2(Nrf2)、血红素加氧酶1(HO-1)、miR-140与氧化应激指标、心功能分级和心室重构的关系。方法选取2020年10月至2022年12月于沧州市人民医院心内三科收治的168例CHF患者作为研究组,另外选择年龄和性别匹配的70名健康人群作为对照组。采用实时荧光定量PCR(RT-qPCR)法测定血清miR-140表达水平,酶联免疫吸附法测定血清Nrf2和HO-1的表达水平以及氧化应激指标,超声心动图检查计算左心室质量指数(LVMI)。结果与对照组相比,研究组CHF患者血清Nrf2和HO-1水平升高,血清miR-140相对表达量降低(P<0.05)。根据受试者工作特征曲线,血清miR-140(AUC=0.803,95%CI:0.734~0.895,P<0.001)、Nrf2(AUC=0.796,95%CI:0.705~0.873,P<0.001)、HO-1(AUC=0.736,95%CI:0.655~0.810,P<0.001)可良好地区分对照组和研究组。CHF患者血清miR-140与血清Nrf2(r=-0.824,P<0.001)、HO-1(r=-0.752,P<0.001)呈负相关性。CHF患者氧化应激指标、心功能指标、LVMI与血清miR-140负相关,与Nrf2、HO-1正相关(P<0.001)。随着心功能分级增加,CHF患者血清Nrf2、HO-1水平逐渐升高,血清miR-140相对表达量逐渐降低(P<0.05)。结论CHF患者中血清miR-140水平降低,同时Nrf2、HO-1水平升高,其表达水平变化可反映患者心功能状态及氧化应激反应,有望成为CHF的候选生物学分子。Objective To discuss the relationship among nuclear factor erythropoietin 2 related factor 2(Nrf2),heme oxygenase 1(HO-1),miR-140,oxidative stress,heart function grading and ventricular remodeling in patients with chronic heart failure(CHF).Methods CHF patients(n=168)were chosen into study group and healthy controls matched by age and sex(n=70)were chosen into control group from the Third Department of Cardiology in People’s Hospital of Cangzhou City from Oct.2020 to Dec.2022.The expression of serum miR-140 was detected by using RT-qPCR.The expressions of serum Nrf2 and HO-1 and oxidative stress indexes were detected by using ELISA.The left ventricular mass index(LVMI)was examined and calculated by using echocardiography.Results The levels of Nrf2 and HO-1 increased,and expression of miR-140 decreased in study group compared with control group(P<0.05).The results of ROC curve analysis showed that AUC of miR-140 was 0.803(95%CI:0.734~0.895,P<0.001)and AUC of Nrf2 was 0.796(95%CI:0.705~0.873,P<0.001).HO-1(AUC=0.736,95%CI:0.655~0.810,P<0.001)could well distinguish control group from study group.The expression of miR-140 was negatively correlated to Nrf2(r=-0.824,P<0.001)and HO-1(r=-0.752,P<0.001)in CHF patients.The oxidative stress indexes,heart function indexes and LVMI were negatively correlated to miR-140 expression,and positively correlated to Nrf2 and HO-1(P<0.001).Along with heart grade increased,the levels of Nrf2 and HO-1 increased gradually,and miR-140 expression decreased gradually(P<0.05).Conclusion The expression of miR-140 decreases and levels of Nrf2 and HO-1 increase in CHF patients,which can reflect heart function and oxidative stress response,and are expected to become candidate biological molecules for CHF.
