基于数据挖掘研究中医内治法降低浆细胞性乳腺炎复发率用药规律

摘　　要：目的:通过古今医案云平台研究中医内治法降低浆细胞性乳腺炎(PCM)复发率的用药规律。方法:以中国知网、万方数据库、维普数据库上已公开发表的文献为基础,收集2012—2022年发表的中医内治法治疗PCM相关文献,建立文献数据库,运用频数统计、聚类分析和关联规则分析等数据挖掘方法分析中药治疗本病的用药规律。结果:共收集文献64篇,剔除使用相同中药处方后,最后纳入56篇文献,得到61首中药处方,共涉及中药118味,用药频数722次,其中使用频数≥20次的中药有7味,分别为柴胡、甘草、蒲公英、当归、赤芍、丹参、白花蛇舌草。药物四气以寒性为主,五味以苦味为主,药物主要归属于肝经,药物功效以清热解毒为主。同时挖掘出14对药对组合和3个聚类方。结论:中药降低PCM复发率用药以苦寒清热为主,主要归肝经。对症治疗以清热解毒消肿为主,可使肿消痛止;对因治疗以疏肝行气、活血化瘀为主,调整体内气滞血瘀的病理状态,预防或减少疾病复发。3首聚类方分别针对气滞血瘀所致的乳痈初起、阳虚寒凝型粉刺性乳痈及粉刺性乳痈溃后正虚邪恋状态。运用数据挖掘技术可总结出中药内服降低PCM复发率的用药规律,为临床优化处方、提高疗效及治疗PCM新方的开发提供参考依据。Objective:To study the medication rules of Chinese medicine internal therapy to reduce the recurrence rate of plasma cell mastitis(PCM) through the Ancient and Modern Medical Records Cloud Platform.Methods:Based on the published literature on CNKI,Wanfang Database and VIP Database,the literature related to using Chinese medicine internal therapy to treat PCM published from 2012 to2022 were collected and the literature database was then established.Data mining methods such as frequency statistics,cluster analysis and association rule analysis were used to analyze the medication rules of Chinese medicine in treating this disease.Results:A total of 64 literature were collected,and 56literature were included after the same Chinese medicine prescriptions were excluded,in which there were totally 61 prescriptions.A total of 118 kinds of Chinese herbs were involved,and the frequency of use was 722 times,among which seven kinds were used twenty or more than twenty times,namely,Bupleuri Radix,Glycyrrhizae Radix et Rhizoma,Taraxaci Herba,Angelicae Sinensis Radix,Paeoniae Radix Rubra,Salviae Miltiorrhizae Radix et Rhizoma,and Herba Hedyotis Diffusae.The four properties of the herbs were mainly cold;the five flavors were mainly bitter;the herbs mainly entered into the liver meridian;the herbal effect was mainly to clear heat and resolve toxins.Meanwhile,fourteen herbal combinations and three clustered prescriptions were obtained.Conclusion:In the medication of using Chinese medicine to reduce the recurrence rate of PCM,bitter-cold herbs are used to clear heat,which mainly enter into the liver meridian.The symptomatic treatment is mainly to clear heat,resolve toxins and disperse swelling,so as to relieve symptoms by dispersing swelling and relieving pain;the causal treatment is mainly to soothe the liver and move qi,invigorate blood and dissolve stasis,so as to adjust the pathological state of qi stagnation and blood stasis in the body to prevent or reduce disease recurrence.The three clustered prescriptions are aimed at the
