百令胶囊联合耳穴贴压对终末期肾病维持性血液透析患者残余肾功能和并发症的影响

摘　　要：目的:观察百令胶囊联合耳穴贴压对终末期肾病(ESRD)维持性血液透析(MHD)患者残余肾功能(RRF)和并发症的影响。方法:选取ESRD 232例患者作为研究对象,根据随机数字表法分为观察组和对照组各116例。对照组予以MHD和常规治疗,观察组在对照组基础上予以百令胶囊联合耳穴贴压治疗。比较2组治疗前后肾功能[尿素氮(BUN)、血清肌酐(SCr)、内生肌酐清除率(CCr)]、营养状况[白蛋白(Alb)、前白蛋白(PA)、血红蛋白(Hb)]、中医证候评分,以及治疗后临床疗效和并发症发生率。结果:治疗后,观察组总有效率为91.38%,对照组为79.31%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组BUN、SCr水平均较治疗前降低(P<0.05),CCr水平较治疗前升高;且观察组BUN、SCr水平均低于对照组(P<0.05),CCr水平高于对照组(P<0.05)。治疗后,2组Alb、PA、Hb水平均较治疗前升高(P<0.05),且观察组上述3项指标均高于对照组(P<0.05)。治疗后,2组倦怠乏力、腰膝酸软、五心烦热、咽干口燥等中医证候评分均较治疗前降低(P<0.05),且观察组上述各项评分均低于对照组(P<0.05)。观察组并发症发生率为6.03%,对照组为10.34%,2组比较,差异无统计学意义(P>0.05)。结论:百令胶囊联合耳穴贴压能有效改善进行MHD的ESRD患者临床症状、营养状况及RRF,且并发症相对较少。Objective:To observe the effect of the therapy of Bailing Capsules combined with auricular point seed-pressing on residual renal function(RRF) and complications in patients with end-stage renal disease(ESRD) on maintenance hemodialysis(MHD).Methods:A total of 232 ESRD patients as the study objects and divided into the observation group and the control group according to random number table method,with 116 cases in each group.The control group was treated with MHD and conventional treatment,and the observation group was additionally treated with Bailing Capsules combined with auricular point seed-pressing based on the treatment of the control group.The indexes of renal function,including blood urea nitrogen(BUN),serum creatinine(SCr) and creatinine clearance(CCr),nutritional status,including albumin(Alb),prealbumin(PA) and hemoglobin(Hb),and traditional Chinese medicine(TCM) syndrome scores before and after treatment,and the clinical effects and complication rates after treatment were compared between the two groups.Results:After treatment,the total effective the was91.38% in the observation group and 79.31% in the control group,the difference being significant(P<0.05).After treatment,the levels of BUN and SCr in the two groups were decreased when compared with those before treatment(P<0.05),and the CCr levels were increased when compared with those before treatment;the levels of BUN and SCr in the observation group were lower than those in the control group(P<0.05),and the CCr level was higher than that in the control group(P<0.05).After treatment,the levels of Alb,PA and Hb in the two groups were increased when compared with those before treatment(P<0.05),and the above three levels in the observation group were higher than those in the control group(P<0.05).After treatment,the scores of TCM syndromes,such as fatigue and lack of strength,waist and knee aching and weakness,vexing heat in the five centers,and dry throat and dry mouth,were decreased in the two groups when compared with those before treatment(P<0.05)
