参芪地黄汤加减内服联合吴茱萸穴位贴敷治疗高血压肾病临床研究

摘　　要：目的:观察参芪地黄汤加减内服联合吴茱萸穴位贴敷治疗高血压肾病(HN)的临床疗效。方法:选取104例HN患者为研究对象,根据计算机程序随机分组法分为对照组与研究组各52例。对照组给予厄贝沙坦片治疗,研究组在对照组基础上加用参芪地黄汤加减联合吴茱萸穴位贴敷治疗。比较2组临床疗效、血压及肾功能指标[尿调节蛋白(UMOD)、肾小球滤过率(eGFR)]、炎症因子[转化生长因子-β1 (TGF-β1)、白细胞介素-6 (IL-6)]水平。结果:研究组总有效率为94.23%,高于对照组80.76%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组舒张压、收缩压均较治疗前降低(P<0.05),且研究组收缩压、舒张压均低于对照组(P<0.05)。治疗后,2组UMOD、eGFR水平均较治疗前升高(P<0.05),且研究组UMOD、eGFR水平均高于对照组(P<0.05)。治疗后,2组TGF-β1、IL-6水平均较治疗前降低(P<0.05),且研究组TGF-β1、IL-6水平均低于对照组(P<0.05)。结论:参芪地黄汤加减联合吴茱萸穴位贴敷治疗HN疗效显著,可缓解临床症状,减轻机体炎症反应,改善血压及肾功能。Objective:To observe the clinical effect of the therapy of oral administration of Modified Shenqi Dihuang Decoction combined with point application with Euodiae Fructus on hypertensive nephrology(HN).Methods:A total of 104 HN patients were selected as the study subjects and divided into the control group and the study group according to the randomized grouping by computer program,with52 cases in each group.The control group was treated with Irbesartan Tablets,and the study group was additionally treated with Modified Shenqi Dihuang Decoction combined with point application with Euodiae Fructus.The clinical effects,blood pressure,and levels of indexes of kidney function,including uromodulin(UMOD) and estimated glomerular filtration rate(eGFR),and inflammatory factors,including transforming growth factor-β1(TGF-β1) and interleukin-6(IL-6),were compared between the two groups.Results:The total effective rate was 94.23% in the study group,higher than that of 80.76% in the control group,the difference being significant(P<0.05).After treatment,the diastolic and systolic pressure in both groups were decreased when compared with those before treatment(P<0.05),and the diastolic and systolic pressure in the study group were lower than those in the control group(P<0.05).After treatment,the levels of UMOD and eGFR in both groups were increased when compared with those before treatment(P<0.05),and the above levels in the study group were higher than those in the control group(P<0.05).After treatment,the levels of TGF-β1 and IL-6 in both groups were decreased when compared with those before treatment(P<0.05),and the above levels in the study group were lower than those in the control group(P<0.05).Conclusion:The therapy of oral administration of Modified Shenqi Dihuang Decoction combined with point application with Euodiae Fructus has a significant curative effect on HN,which can mitigate the clinical symptoms and inflammatory responses of the body,and improve the blood pressure and kidney function.
