舌下含服粉尘螨滴剂对变应性鼻炎患者的临床疗效评估

摘　　要：目的评估舌下含服粉尘螨滴剂对变应性鼻炎(AR)患者的临床疗效。方法选取2019年6月至2021年11月在南通市海门区人民医院耳鼻喉科就诊的76例AR患者为研究对象,根据皮肤点刺试验结果分为单一致敏组(30例)和多重致敏组(46例),两组均舌下含服粉尘螨滴剂进行舌下特异性免疫治疗至少6个月以上。比较两组患者治疗前、治疗1个月、3个月、6个月后的鼻部症状(喷嚏、流涕、鼻塞、鼻痒)评分及鼻部症状总评分(TNSS)、药物总评分(TMS)、视觉模拟评分法(VAS)评分。结果治疗3个月后,多重致敏组的喷嚏评分显著低于单一致敏组(P<0.01);治疗1个月后,多重致敏组的鼻痒评分显著低于单一致敏组(P<0.05)。治疗3个月后,多重致敏组的TNSS、TMS、VAS评分均低于单一致敏组[3.50(2.00,5.00)分比6.00(2.75,7.00)分,0.00(0.00,0.00)分比1.00(0.00,1.00)分,3.00(2.00,5.00)分比6.00(3.75,7.25)分](P<0.01);治疗1个月后,多重致敏组的VAS评分低于单一致敏组[6.00(4.00,7.00)分比6.50(5.00,8.00)分](P<0.05)。结论舌下含服粉尘螨滴剂对单一致敏和多重致敏AR患者均有效,且多重致敏AR患者相较单一致敏AR患者各项指标缓解更加迅速。Objective To evaluate the clinical efficacy of sublingual administration of dust mite drops in patients with allergic rhinitis(AR).Methods A total of 76 AR patients who visited Department of Otolaryngology of Nantong Haimen People′s Hospital from Jun.2019 to Nov.2021 were included.They were divided into a single sensitization group(30 cases)and a multiple sensitization group(46 cases)based on the results of skin prick test.Both groups received sublingual specific immunotherapy with dust mite drops for at least 6 months.The scores of nasal symptoms(sneezing,runny nose,nasal congestion,and itching),as well as the total nasal symptom score(TNSS),total medications score(TMS),and visual analogue scale(VAS)score before treatment,1 month,3 months,and 6 months after treatment of the two groups were compared.Results After 3 months of treatment,the sneezing scores in the multiple sensitization group was significantly lower than that in the single sensitization group(P<0.01);after one month of treatment,the nasal itching scores in the multiple sensitization group was significantly lower than that in the single sensitization group(P<0.05).After 3 months of treatment,the TNSS,TMS,and VAS scores of the multiple sensitization group were lower than those of the single sensitization group[3.50(2.00,5.00)vs 6.00(2.75,7.00),0.00(0.00,0.00)vs 1.00(0.00,1.00),3.00(2.00,5.00)vs 6.00(3.75,7.25)](P<0.01);after one month of treatment,the VAS score of the multiple sensitization group was lower than that of the single sensitization group[6.00(4.00,7.00)vs 6.50(5.00,8.00)](P<0.05).Conclusion The sublingual administration of dust mite drops is effective for both single and multiple sensitized AR patients,and multiple sensitized AR patients have more rapid relief of various indicators compared to single sensitized AR patients.
