聊城市HIV感染者/AIDS患者CD4+T淋巴细胞计数和病毒载量检测结果分析

摘　　要：目的分析聊城市在治的HIV感染者/AIDS患者(以下简称HIV/AIDS)CD4+T淋巴细胞计数和病毒载量检测结果,为制定HIV/AIDS防控措施提供科学依据。方法在中国疾病预防控制传染病监测信息系统下载现住址为聊城市、治疗时间半年以上的HIV/AIDS患者个案信息,由市疾病预防控制中心(以下简称疾控中心)负责对各县(市、区)疾控中心及相关医疗机构送检的患者血液标本进行检测。结果聊城市775例HIV/AIDS中,CD4+T淋巴细胞计数<200个/μL、200~<500个/μL、≥500个/μL的患者分别占10.45%、44.00%、45.55%。556例患者未检出病毒,占71.74%;病毒载量<1000拷贝/mL、≥1000拷贝/mL的患者为分别占19.87%、8.39%。趋势χ^(2)检验结果显示,治疗时间越长,CD4+T淋巴细胞计数≥500个/μL的患者越多,未检出HIV病毒的患者也越多。经过治疗后,病毒复制速度减缓,免疫功能增强,病毒载量与CD4+T淋巴细胞计数呈负相关(r=-0.034,P<0.01)。结论聊城市HIV/AIDS抗病毒治疗效果明显。对部分患者治疗失败的情况应予以高度关注,要加强对HIV/AIDS患者的心理干预,提高治疗成功率。Objective To analyze the results of the CD4+T lymphocyte counting and viral loading test of HIV/AIDS patients under treatment in Liaocheng City,and provide scientific basis for formulating prevention and control measures.Methods Downloading patients’information from China Disease Prevention and Control Infectious Disease Monitoring Information System with current address in Liaocheng City.Liaocheng CDC was responsible for testing the blood samples of patients submitted by the CDCs in various counties (cities,districts) and relevant medical institutions of Liaocheng City.Results Among the 775 HIV/AIDS cases in Liaocheng City,CD4+T lymphocyte counts<200/μL,200~<500/μL and≥500/μL,accounted for 10.45%,44.00%and 45.55%,respectively.556 cases were unable to detect the virus,accounting for 71.74%.The number of patients with viral load<1 000 copies/mL,≥1 000 copies/mL,accounted for19.87%and 8.39%,respectively.Trendχ^(2)test analysis showed that the longer the treatment time was,the more patients had CD4+T lymphocyte counts≥500/μL,and the more patients could not detect HIV virus.After treatment,the virus replication rate slowed a down trend,immune function increased,and virus load was negatively correlated with CD4+T lymphocyte count (r=-0.034,P<0.01).Conclusion The effectiveness of antiviral treatment for HIV/AIDS in Liaocheng is significant.High attention should be paid to the failure of treatment in some patients,and psychological intervention should be strengthened for HIV/AIDS patients,so as to improve the success rate of treatment.
