摘 要:目的比较异丙酚静脉麻醉与异氟醚吸入麻醉在低温失血性休克猪模型中的麻醉效果。方法选取12头雄性巴马小型猪,采用随机数字法分为两组,每组6头。Ⅰ组用异丙酚静脉麻醉,Ⅱ组用异氟醚吸入麻醉,在-30℃寒仓中,建立失血性休克猪模型。连续监测实验猪的心率(HR)、平均动脉压(MAP)、脉搏血氧饱和度(SPO_(2))及PH值的变化。记录麻醉时间、自主呼吸恢复时间。结果两组的放血量比较,差异无统计学意义(P>0.05)。Ⅱ组麻醉时间、自主呼吸恢复时间均短于Ⅰ组,差异有统计学意义(P<0.05)。两组均未出现恶心、呕吐等不良反应。两组造模前HR、MAP、SPO_(2)及PH值比较,差异无统计学意义(P>0.05)。造模后,两组MAP显著低于造模前,而Ⅱ组MAP高于Ⅰ组,差异有统计学意义(P<0.05)。造模后,两组HR显著高于造模前,而Ⅱ组HR低于Ⅰ组,差异有统计学意义(P<0.05)。结论与异丙酚静脉麻醉比较,异氟醚吸入麻醉的麻醉过程较平稳,麻醉效果好,且术后苏醒快,能够较好维持血流动力学的稳定。Objective To compare the anesthetic effect of propofol intravenous anesthesia and isoflurane inhalation anesthesia in hypothermia hemorrhagic shock pig model.Methods 12 male Bama miniature pigs were selected and divided into two groups with 6 pigs per group by random number method.GroupⅠwas given intravenous anesthesia with propofol,groupⅡwas given inhalation anesthesia with isoflurane,and a hemorrhagic shock pig model was established in a cold chamber at-30℃.The changes of heart rate(HR),mean arterial pressure(MAP),pulse oxygen saturation(SPO_(2))and PH value were continuously monitored.Anesthesia time and recovery time of spontaneous respiration were recorded.Results There was no significant difference in blood release between the two groups(P>0.05).The time of anesthesia and recovery time of spontaneous respiration in groupⅡwere shorter than those in groupⅠ,and the difference was statistically significant(P<0.05).No adverse reactions such as nausea and vomiting occurred in both groups.There was no significant difference in HR,MAP,SPO_(2) and PH values between the two groups before modeling(P>0.05).After modeling,the MAP of the two groups was significantly lower than that before modeling,while the MAP of the groupⅡwas higher than that of the groupⅠ,with statistical significance(P<0.05).After modeling,HR in both groups was significantly higher than that before modeling,while HR in groupⅡwas lower than that in groupⅠ,the difference was statistically significant(P<0.05).Conclusion Compared with intravenous anesthesia with propofol,inhalation anesthesia with isoflurane has a more stable anesthetic process,better anesthetic effect,faster postoperative recovery,and better hemodynamic stability.
