摘　　要：目的探讨经腘静脉入路介入治疗联合利伐沙班用于血液透析下肢深静脉血栓的效果及对凝血纤溶功能的影响。方法选取2018年1月至2022年1月收治的60例留置股静脉导管的血液透析下肢深静脉血栓患者为研究对象,根据治疗方法的差异将其分为对照组(30例,利伐沙班治疗)和观察组(30例,经腘静脉入路介入治疗联合利伐沙班治疗)。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的大腿、小腿周径差值均低于对照组(P<0.05)。治疗后,观察组的凝血酶原时间(PT)、活化部分凝血酶时间(APTT)长于对照组,纤维蛋白原(FIB)、血栓调节蛋白(TM)、组织型纤溶酶原激活物-抑制剂复合物(t-PAIC)、纤溶酶-抗纤溶酶复合物(PIC)水平低于对照组(P<0.05)。结论经腘静脉入路介入治疗联合利伐沙班用于血液透析下肢深静脉血栓患者可提高治疗效果,减轻下肢肿胀,促进凝血、纤溶功能改善,值得推广。Objective To investigate the effect of interventional therapy through popliteal vein approach combined with rivaroxaban on deep venous thrombosis of lower extremity in hemodialysis and its influences on coagulation and fibrinolysis functions.Methods From January 2018 to January 2022,60 hemodialysis patients with deep venous thrombosis of lower extremity undergoing indwelling femoral vein catheter were selected as the research objects.According to the difference of treatment methods,the patients were divided into control group(30 cases,rivaroxaban treatment)and observation group(30 cases,interventional therapy through popliteal vein approach combined with rivaroxaban treatment).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the circumference differences of the thigh and calf in the observation group were lower than those in the control group(P<0.05).After treatment,the prothrombin time(PT)and activated partial thromboplastin time(APTT)in the observation group were longer than those in the control group,and the levels of fibrinogen(FIB),thrombomodulin(TM),tissue plasminogen activator-plasminogen activator inhibitor-1 complex(t-PAIC)and plasmin antiplasmin complex(PIC)were lower than those in the control group(P<0.05).Conclusion Interventional therapy through popliteal vein approach combined with rivaroxaban on deep venous thrombosis of lower extremity in hemodialysis can improve the therapeutic effect,reduce the swelling of lower extremities,and promote the improvement of coagulation and fibrinolysis functions,which is worthy of promotion.
