超声与CT在痛风患者尿酸钠晶体沉积中的诊断价值

摘　　要：目的比较超声与CT对痛风患者尿酸钠晶体沉积的诊断价值。方法选取2020年3月至2022年3月收治的86例痛风患者为研究对象。所有患者均行超声、CT及病理检查,以病理检查为金标准,比较超声与CT对痛风患者尿酸钠晶体沉积的诊断价值,分析不同时期痛风患者的主要超声征象。结果86例痛风患者共检查121个部位,经病理检查有72个部位检出尿酸钠晶体沉积阳性。经超声扫描有69个部位出现阳性,经CT扫描有70个部位出现阳性,超声与CT对尿酸钠晶体沉积的诊断效能无显著差异(P>0.05)。无症状高尿酸血症期患者的超声征象多为阴性;急性期及慢性期患者的超声征象多为阳性,以双轨征和痛风石为主。结论超声与CT对痛风患者尿酸钠晶体沉积均有一定的临床诊断价值,在首选诊断手段中超声更具优势;超声征象阳性多见于急性期及慢性期痛风患者,双轨征和痛风石是常见的超声征象。Objective To compare the diagnostic value of ultrasound and CT for sodium urate crystal deposition in patients with gout.Methods Eighty six patients with gout admitted from March 2020 to March 2022 were selected as the research objects.All patients underwent ultrasound,CT and pathological examination.With pathological examination as the gold standard,the diagnostic value of ultrasound and CT for sodium urate crystal deposition in patients with gout was compared,and the main ultrasonic signs of gout patients in different periods were analyzed.Results A total of 121 joints were examined in 86 patients with gout,and 72 joints were found to be positive for sodium urate crystal deposition by pathological examination.There were 69 positive joints by ultrasound scanning and 70 positive joints by CT scanning.There was no significant difference in the diagnostic efficacy of ultrasound and CT for sodium urate crystal deposition(P>0.05).The ultrasonic signs of patients with asymptomatic hyperuricemia were mostly negative;the ultrasonic signs of patients in acute and chronic stages were mostly positive,mainly double-track sign and tophi.Conclusion Ultrasound and CT have certain clinical diagnostic value for sodium urate crystal deposition in patients with gout,and ultrasound has more advantages in the first choice of diagnostic methods;positive ultrasound signs are more common in patients with acute and chronic gout,and double-track sign and tophi are common ultrasound signs.
