摘 要:目的分析胱抑素C(CysC)、淀粉样蛋白A(SAA)与β_(2)-微球蛋白(β_(2)-MG)联合检验对早期糖尿病肾病(DN)的诊断价值。方法选取48例早期DN患者作为观察组,另选取46例单纯2型糖尿病患者作为对照组。两组患者均接受CysC、SAA、β_(2)-MG、尿微量白蛋白(mAlb)水平检测。比较两组患者CysC、SAA、β_(2)-MG、mAlb水平;比较CysC、SAA、β_(2)-MG单独检验与三项指标联合检验对早期DN的诊断灵敏度与特异度;分析早期DN患者CysC、SAA、β_(2)-MG与mAlb的相关性。结果观察组CysC(1.83±0.25)mg/L、SAA(15.85±1.23)mg/L、β_(2)-MG(0.65±0.12)mg/L、mAlb(75.50±8.48)均高于对照组的(0.38±0.18)、(5.73±0.56)、(0.15±0.08)、(20.05±2.00)mg/L,差异有统计学意义(P<0.05)。CysC单独检验对早期DN的诊断灵敏度与特异度分别为68.75%与67.39%,SAA单独检验对早期DN的诊断灵敏度与特异度分别为75.00%与76.09%,β_(2)-MG单独检验对早期DN的诊断灵敏度与特异度分别为60.42%、60.87%,三项指标联合检验对早期DN的诊断灵敏度与特异度分别为95.83%与95.65%;三项指标联合检验对早期DN的诊断灵敏度与特异度均高于CysC、SAA、β_(2)-MG单独检验(P<0.05)。Pearson相关性分析发现,CysC、SAA、β_(2)-MG与早期DN患者mAlb呈正相关性(r=0.615、0.895、0.590,P<0.05)。结论CysC、SAA与β_(2)-MG联合检验在早期DN诊断中具有较高临床应用价值,灵敏度与特异度更高,适于临床推广。Objective To analyze the value of combined detection of Cystatin C(CysC),serum amyloid A(SAA)andβ_(2)-microglobulin(β_(2)-MG)in the diagnosis of early diabetes nephropathy(DN).Methods 48 patients with early DN were taken as the study group,and another 46 patients with simple type 2 diabetes mellitus as the control group.Both groups of patients received test of CysC,SAAβ_(2)-MG,microalbuminuria(mAlb).The levels of CysC,SAA,β_(2)-MG and mAlb were compared between the two groups.The diagnostic sensitivity and specificity of individual and combined test of CysC,SAA andβ_(2)-MG for early DN were compared.The correlation of CysC,SAA,β_(2)-MG with mAlb in patients with early DN was analyzed.Results The observation group had CysC of(1.83±0.25)mg/L,SAA of(15.85±1.23)mg/L,β_(2)-MG of(0.65±0.12)mg/L,mAlb of(75.50±8.48),which were higher than(0.38±0.18),(5.73±0.56),(0.15±0.08),(20.05±2.00)mg/L in the control group,and the difference was statistically significant(P<0.05).The diagnostic sensitivity and specificity of CysC test alone for early DN test were 68.75%and 67.39%;the diagnostic sensitivity and specificity of SAA test alone were 75.00%and 76.09%;the sensitivity and specificity ofβ_(2)-MG test alone were 60.42%and 60.87%;the diagnostic sensitivity and specificity of combined test for early DN were 95.83%and 95.65%.The diagnostic sensitivity and specificity of the combined test for early DN were higher than those of CysC,SAA andβ_(2)-MG test alone(P<0.05).Pearson correlation analysis found that there was a positive correlation of CysC,SAAβ_(2)-MG with mAlb in patients with early DN(r=0.615,0.895,0.590;P<0.05).Conclusion CysC,SAA andβ_(2)-MG has application value in early diagnosis of DN,and the sensitivity and specificity of combined test are higher,which is suitable for clinical promotion.
