盐酸贝那普利片联合苯磺酸氨氯地平片治疗老年高血压的临床效果

摘　　要：目的探究盐酸贝那普利片联合苯磺酸氨氯地平片治疗老年高血压的效果。方法800例老年高血压患者,根据数字法分为观察组(500例)和对照组(300例)。观察组采用盐酸贝那普利片联合苯磺酸氨氯地平片治疗,对照组采用苯磺酸氨氯地平片联合厄贝沙坦片治疗。比较两组患者临床疗效、不良反应发生情况以及治疗前后血压(收缩压、舒张压)、肾功能指标(血清肌酐、血尿素氮、尿微量白蛋白)、血压24 h波动值。结果观察组治疗总有效率97.20%与对照组的95.00%比较无明显差异(P>0.05)。治疗后两组收缩压、舒张压均低于治疗前(P<0.05)。治疗后,观察组患者血清肌酐(88.94±7.62)μmol/L、血尿素氮(4.72±0.36)mmol/L、尿微量白蛋白(58.41±5.51)mg/24 h均低于对照组的(93.62±6.72)μmol/L、(5.09±0.42)mmol/L、(84.33±3.77)mg/24 h(P<0.05)。观察组不良反应发生率2.80%低于对照组的10.00%(P<0.05)。治疗后,观察组收缩压24 h波动值(8.23±1.31)mm Hg(1 mm Hg=0.133 kPa)、舒张压24 h波动值(5.42±1.41)mm Hg均低于对照组的(10.42±1.56)、(8.72±1.68)mm Hg(P<0.05)。结论盐酸贝那普利片联合苯磺酸氨氯地平片治疗老年高血压的疗效较好,值得应用。Objective To explore the effect of benazepril hydrochloride tablets combined with amlodipine besylate tablets in the treatment of senile hypertension.Methods 800 cases of senile hypertension were divided into an observation group(500 cases)and a control group(300 cases)according to numerical method.The observation group was treated with benazepril hydrochloride tablet and amlodipine besylate tablet,and the control group was treated with amlodipine besylate tablet and irbesartan tablet.Comparison was made on clinical efficacy,occurrence of adverse reactions,blood pressure(systolic blood pressure,diastolic blood pressure),renal function index(serum creatinine,blood urea nitrogen,urinary microalbumin)and fluctuation of 24-h blood pressure before and after treatment between the two groups.Results The total effective rate of the observation group was 97.20%,and the difference was not significant compared with 95.00%of the control group(P>0.05).The systolic blood pressure and diastolic blood pressure of the two groups after treatment were lower than those before treatment(P<0.05).After treatment,the observation group had serum creatinine of(88.94±7.62)μmol/L,blood urea nitrogen of(4.72±0.36)mmol/L,and urinary microalbumin of(58.41±5.51)mg/24 h,which were lower than(93.62±6.72)μmol/L,(5.09±0.42)mmol/L and(84.33±3.77)mg/24 h in the control group(P<0.05).The incidence of adverse reactions in the observation group was 2.80%,which was lower than 10.00%in the control group(P<0.05).After treatment,the observation group had fluctuation of 24-h systolic blood pressure of(8.23±1.31)mm Hg(1 mm Hg=0.133 kPa)and fluctuation of 24-h diastolic blood pressure of(5.42±1.41)mm Hg,which were lower than(10.42±1.56)and(8.72±1.68)mm Hg in the control group(P<0.05).Conclusion Benazepril hydrochloride tablets combined with amlodipine besylate tablets has good curative effect in the treatment of senile hypertension and is worthy of application.
