糖皮质激素吸入联合脾氨肽在过敏性鼻炎-哮喘综合征患儿中的应用研究

摘　　要：目的分析糖皮质激素吸入联合脾氨肽在过敏性鼻炎-哮喘综合征(CARAS)患儿中的应用效果。方法选取90例CARAS患儿,采用随机数字表法分为对照组(n=45)与研究组(n=45)。对照组患儿采用糖皮质激素吸入治疗,研究组在对照组基础上应用脾氨肽治疗。比较两组患儿的临床疗效,治疗前后炎症因子[白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)]、免疫因子[免疫球蛋白M(IgM)、免疫球蛋白G(IgG)]以及不良反应发生情况。结果研究组治疗总有效率95.56%较对照组的80.00%高(P<0.05)。治疗3个月后,研究组IL-4(40.65±5.89)pg/ml、hs-CRP(9.52±2.57)mg/L均较对照组的(49.60±5.85)pg/ml、(12.86±3.00)mg/L低(P<0.05)。治疗3个月后,研究组IgG(6.85±1.02)g/L、IgM(2.90±0.59)g/L均较对照组的(5.94±1.00)、(2.30±0.45)g/L高(P<0.05)。两组不良反应发生率比较,结果无差异性(χ^(2)=0.450,P=0.502>0.05)。结论糖皮质激素吸入联合脾氨肽治疗儿童CARAS效果确切,可以有效抑制机体炎症反应,增强其免疫功能。Objective To analyze the application effect of glucocorticoid inhalation combined with spleen aminopeptide in children with combined allergic rhinitis and asthma syndrome(CARAS).Methods 90 children with CARAS were divided into a control group(n=45)and a study group(n=45)according to random numerical table.The control group was treated with glucocorticoid inhalation,while the study group was treated with spleen aminopeptide on the basis of the control group.The clinical efficacy,inflammatory factors[interleukin-6(IL-6),hypersensitive C-reactive protein(hs-CRP)]and immune factors[immunoglobulin M(IgM),immunoglobulin G(IgG)]before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of 95.56%in the study group was higher than 80.00%in the control group(P<0.05).After 3 months of treatment,the study group had IL-4 of(40.65±5.89)pg/ml and hs-CRP of(9.52±2.57)mg/L,which were lower than(49.60±5.85)pg/ml and(12.86±3.00)mg/L in the control group(P<0.05).After 3 months of treatment,the study group had IgG of(6.85±1.02)g/L and IgM of(2.90±0.59)g/L,which were higher than(5.94±1.00)and(2.30±0.45)g/L in the control group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(χ^(2)=0.450,P=0.502>0.05).Conclusion Glucocorticoid inhalation combined with spleen aminopeptide is effective in the treatment of CARAS,which can effectively inhibit the inflammatory reaction of children,enhance their immune function.
