摘　　要：目的 探讨CO_(2)点阵激光联合窄谱中波紫外线(NB-UVB)治疗非节段型白癜风(NSV)的效果及对心身状态的影响。方法 选取2022年1月至2023年1月徐州市矿务集团总医院皮肤科门诊治疗的70例NSV患者,采用随机数表分为NB-UVB组(n=35,NB-UVB治疗)和联合组(n=35,CO_(2)点阵激光联合NB-UVB治疗)。比较两组临床疗效,白癜风面积及严重程度(VASI)评分,肿瘤坏死因子(TNF-α)、γ-干扰素(IFN-γ)和白细胞介素-4(IL-4)水平,T淋巴细胞亚群(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平,以及临床症状自评量表(SCL-90)评分。结果 联合组总有效率高于NB-UVB组,差异有统计学意义(P<0.05)。治疗后,联合组VASI、SCL-90评分以及TNF-α、IFN-γ、CD8^(+)水平低于NB-UVB组,差异有统计学意义(P<0.05);且联合组IL-4、CD4^(+)、CD4^(+)/CD8^(+)水平高于NB-UVB组,差异有统计学意义(P<0.05)。结论 CO_(2)点阵激光联合NB-UVB治疗NSV的效果显著,能抑制炎症反应,调节免疫平衡,改善心身状态。Objective To explore the effect of CO_(2) fractional laser combined with narrow-band ultraviolet B (NB-UVB) in the treatment of non-segmental vitiligo (NSV) and its impact on the psychosomatic state.Methods A total of 70 NSV patients who were treated at the Dermatology Clinic of the General Hospital of Xuzhou Mining Group from January 2022 to January 2023 were divided into an NB-UVB group (n=35,receiving NB-UVB treatment) and a combination group (n=35,receiving CO_(2) fractional laser combined with NB-UVB treatment) using the random number table method.The clinical efficacy,vitiligo area severity index(VASI),levels of tumor necrosis factor-α (TNF-α),γ-interferon (IFN-γ) and interleukin-4 (IL-4),levels of T-lymphocyte subsets (CD4^(+) ,CD8^(+) ,CD4^(+) /CD8^(+) ),and clinical symptom checklist 90 (SCL-90) scores of the two groups were compared.Results The total effective rate of the combination group was higher than that of the NB-UVB group,with statistically significant differences (P<0.05).After treatment,VASI,SCL-90 scores,and the levels of TNF-α,IFN-γ,and CD8^(+) in the combination group were lower than those in the NB-UVB group,with statistically significant differences (P<0.05).The levels of IL-4,CD4^(+) and CD4^(+) /CD8^(+) in the combination group were higher than those in the NB-UVB group,with statistically significant differences (P<0.05).Conclusion The CO_(2) fractional laser combined with NB-UVB has a significant effect on the treatment of NSV,which can inhibit inflammatory reactions,regulate immune balance,and improve the psychosomatic state.
