摘 要:目的研究活动性肺结核合并糖尿病患者血糖水平与树突状细胞(DCs)亚群及T细胞亚群的相关性。方法选取2020年1月至2022年1月惠州市中心人民医院接受诊治的200例患者为研究对象,按照病情分为单纯活动性肺结核患者(对照组)和活动性肺结核合并糖尿病患者(研究组),每组各100例。按照患者糖化血红蛋白(HbA1c)水平将研究组患者分为轻度高血糖组(10 mmol/L≤HbA1c<13.9 mmol/L)45例、中度高血糖组(13.9 mmol/L≤HbA1c<22 mmol/L)33例、重度高血糖组(HbA1c≥22 mmol/L)22例。比较每组患者血糖指标、DCs亚群、T细胞亚群、细胞因子水平,并分析不同血糖程度与DCs、T细胞亚群的相关性。结果四组患者各血糖指标比较为对照组<轻度高血糖组<中度高血糖组<重度高血糖组(P<0.05);DCs亚群(cDC1、cDC2、pDC)及T细胞亚群(CD4^(+)、CD8^(+)、CD3^(+))分布值比较为对照组>轻度高血糖组>中度高血糖组>重度高血糖组(P<0.05);四组患者各炎性细胞因子水平比较为对照组<轻度高血糖组<中度高血糖组<重度高血糖组(P<0.05);外周血DCs亚群、T细胞亚群分布情况与血糖水平呈负相关(P<0.05)。结论针对活动性肺结核合并糖尿病患者,不同血糖程度患者炎性细胞因子水平、DCs亚群及T细胞亚群分布情况不同,且DCs亚群及T细胞亚群分布情况与患者血糖水平呈负相关。Objective To study the correlation between blood glucose level and dendritic cells(DCs)subsets and T cell subsets in patients with active pulmonary tuberculosis complicated with diabetes mellitus.Methods A total of 200 patients diagnosed and treated in Huizhou Gentral People’s Hospital from January 2020 to January 2022 were selected as the clinical study subjects.According to their condition,they were divided into the control group(simple active pulmonary tuberculosis patients),and the study group(active pulmonary tuberculosis patients complicated with diabetes mellitus),with 100 patients in each group.T he patients in the s tudy group were divided into the mild hyperglycemia group(10 mmol/L≤HbA1c<13.9 mmol/L)(n=45),the moderate hyperglycemia group(13.9 mmol/L≤HbA1c<22 mmol/L)(n=33),and the severe hyperglycemia group(HbA1c≥22mmol/L)(n=22)according to the level of glycosylated hemoglobin(HbA1c).The blood glucose parameters,comparison of T cell subsets,cytokine levels,and the correlation between different blood glucose levels and DCs and T cell subsets in each group of patients were observed.Results The comparison of blood glucose indices among the four groups of patients was as follows:control group<mild hyperglycemia group<moderate hyperglycemia group<severe hyperglycemia group(P<0.05).The distribution values of DCs subsets(cDC1,cDC2,pDC)and T cell subsets(CD4^(+),CD8^(+),CD3^(+))in the four groups of patients were compared as follows:control group>mild hyperglycemia group>moderate hyperglycemia group>severe hyperglycemia group(P<0.05).The comparison of inflammatory cytokine levels among the four groups of patients was as follows:control group<mild hyperglycemia group<moderate hyperglycemia group<severe hyperglycemia group(P<0.05).The distribution of DCs subsets and T cell subsets in peripheral blood was negatively correlated with blood glucose(P<0.05).Conclusion For patients with active pulmonary tuberculosis complicated with diabetes mellitus,the levels of inflammatory cytokines,the distribution of DCs
