摘　　要：目的:分析喉罩全身麻醉联合超声引导下神经阻滞在骨科老年患者髋部手术中的应用效果。方法:选取2019年3月—2022年3月于北京大望路急诊抢救医院骨科行髋部手术的老年患者90例作为研究对象,按随机数字表法分为对照组与观察组,各45例。对照组给予常规静吸麻醉联合喉罩全身麻醉,观察组给予喉罩全身麻醉联合超声引导下神经阻滞。比较两组麻醉效果。结果:观察组喉罩置入即刻、切皮时、麻醉后30 min、麻醉后60 min、手术完成时心率、血压低于对照组,麻醉起效时间、苏醒时间、拔管时间、下床活动时间早于对照组,且术后视觉模拟评分、谵妄及认知障碍发生率低于对照组,差异有统计学意义(P<0.05)。结论:喉罩全身麻醉联合超声引导下神经阻滞在骨科老年患者髋部手术中的应用效果较好,值得推广。Objective:To analyze the application effect of laryngeal mask general anesthesia combined with ultrasound-guided nerve block in hip surgery of elderly orthopedic patients.Methods:Ninety cases of elderly patients who underwent hip surgery in the department of orthopedics of Beijing Dawanglu Emergency Rescue Hospital from March 2019 to March 2022 were selected as the study subjects,and were divided into the control group and the observation group according to the method of randomized numerical table,each of which was 45 cases.The control group was given conventional intravenous inhalation anesthesia combined with laryngeal mask general anesthesia,and the observation group was given laryngeal mask general anesthesia combined with ultrasound-guided nerve block.The anesthetic effect of the two groups was compared.Results:The heart rate and blood pressure of the observation group were lower than those of the control group at the moment of laryngeal mask placement,at the time of skin incision,30 min after anesthesia,60 min after anesthesia,and at the completion of surgery,the onset time of anesthesia,recovery time,extubation time and ambulation time in the observation group were earlier than those in the control group,and the postoperative visual analogue score,delirium and cognitive impairment incidence in the observation group were lower than those in the control group,with a statistically significant difference(P<0.05).Conclusion:Laryngeal mask general anesthesia combined with ultrasound-guided nerve block has a good application effect in hip surgery of elderly orthopedic patients,and it is worth promoting.
