温肾化痰祛瘀方治疗良性高血压性肾硬化临床疗效研究

摘　　要：目的:研究温肾化痰祛瘀方治疗良性高血压性肾硬化患者的临床疗效,旨在为当前治疗良性高血压肾损害提供中西医结合诊疗思路。方法:招募2020年01月—2020年12月就诊的良性高血压肾损害患者共70例,完成随访病例59例,采用随机双盲对照原则,根据随机数字对照表分成两组,治疗组在一般治疗基础上口服温肾化痰祛瘀方,对照组则联合中药安慰剂,疗程6个月,检测治疗前后肌酐(serumcreatinine,Scr)、肾小球滤过率(glomerular filtration rate,GFR)、尿微量白蛋白(micro albumin,MA)、24 h尿蛋白定量(24 hour urine total protein,24 h UTP)等指标。结果:治疗组较对照组降低Scr,提高GFR的结果更为显著,差异有统计学意义(P<0.05);与对照组比较,治疗组对于改善尿微量蛋白及尿NAG酶排泄更为明显(P<0.05)。结论:温肾化痰祛瘀方联合缬沙坦治疗良性高血压肾硬化较单纯缬沙坦可明显改善肾功能,改善尿蛋白排泄和脂质代谢,延缓慢性肾脏病进展。Objective:To study the clinical efficacy of Wenshen Huatan Quyu Recipe in the treatment of patients with Be⁃nign Hypertensive Nephrosclerosis,aiming to provide integrated Chinese and Western medicine diagnosis and treatment ideas for the current treatment of Benign Hypertensive Nephrosclerosis.Methods:Seventy outpatients and inpatients of Shanghai Yueyang Hospital were selected to meet the inclusion criteria for benign hypertension and kidney damage.They came from Yueyang Hospital from Janu⁃ary 2020 to December 2020.They were divided into two groups according to the randomized number control table based on the princi⁃ple of randomized double-blind control.The treatment group received Wenshen Huatan Quyu Recipe on based on the basic treat⁃ment,twice a day for 6 months;the control group received the placebo of Wenshen Huatan Quyu Recipe on based on the basic treat⁃ment,twice a day for the course of treatment 6 months.Serum creatinine(Scr),Glomerular filtration rate(eGFR),urine microalbu⁃min(UMA),24 hour urine protein total(24 h UTP)were measured.Results:The treatment group decreased the level of Scr、UMA、UNAG(P<0.05)compared with the control group.The treatment group increased the level of eGFR(P<0.05)compared with the control group.Conclusion:The combination of Wenshen Huatan Quyu with valsartan in the treatment of benign hypertensive nephrosclerosis can significantly improve renal function,urinary protein excretion,and delay the further progress of kidney disease compared with valsartan alone.
