消栓止痛胶囊联合补阳还五汤治疗肢体动脉硬化闭塞症患者的疗效观察

摘　　要：目的探究消栓止痛胶囊联合补阳还五汤治疗肢体动脉硬化闭塞症患者的临床疗效。方法选取2017年1月—2022年6月期间河北省石家庄市中医院脉管科收治的肢体动脉硬化闭塞症患者90例作为研究对象,按随机数字表法分为对照组和研究组,每组各45例。所有受试者均予以西医外科腹主动脉-股主动脉穿刺的常规治疗,对照组联合补阳还五汤治疗,研究组在对照组基础上联合消栓止痛胶囊治疗。治疗2周后,观察比较两组患者临床疗效、不良反应发生情况,治疗前后中医证候评分、血流速度指标[平均血流速度和血流频谱峰值]、血清生化指标[尿酸(Uric acid,UA)和超敏反应蛋白(high-sensitivity c-reactive protein,hs-CRP)]水平。结果治疗后两组患者中医证候积分较治疗前降低,差异有统计学意义(P<0.05);且研究组中医证候积分明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者平均血流速度、血流频谱峰值均较治疗前升高,差异有统计学意义(P<0.05);且研究组平均血流速度、血流频谱峰值均明显高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者血清生化指标hs-CRP、UA水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组血清生化指标hs-CRP、UA水平均明显低于对照组,差异有统计学意义(P<0.05)。治疗后研究组临床总有效率95.56%(43/45)明显高于对照组82.22%(37/45),差异有统计学意义(χ^(2)=4.050,P<0.05)。治疗期间,两组患者均未出现严重不良反应。结论消栓止痛胶囊联合补阳还五汤治疗肢体动脉硬化闭塞症,有助于改善患者的血流动力学,调节机体代谢反应,抑制炎症反应,临床疗效优越且安全性良好。Objective To evaluate the clinical efficacy of Xiaoshuan Zhitong Capsules combined with Buyang Huanwu Decoction in treating patients with arteriosclerosis obliterans of limbs.Methods Ninety patients diagnosed with arteriosclerosis obliterans of limbs at the Vascular Department of Shijiazhuang Hospital of Traditional Chinese Medicine in Hebei Province from January 2017 to June 2022 were selected as study participants.They were divided into a control group and a study group by random number table,with 45 in each group.All patients received routine western surgical intervention with abdominal aorta-to-femoral artery puncture.The control group was given Buyang Huanwu Decoction,while the study group was given Xiaoshuan Zhitong Capsules on the basis of the control group.After two weeks of treatment,clinical efficacy,adverse reactions,traditional Chinese medicine(TCM)syndrome scores,blood flow velocity indices(mean blood flow velocity and peak flow spectrum),and serum biochemical markers[uric acid(UA)and high-sensitivity C-reactive protein(hs-CRP)]were assessed.Results The two groups had a reduction in TCM syndrome scores(P<0.05)after treatment,with the study group exhibiting lower scores than the control group(P<0.05).Similarly,improvements in mean blood flow velocity and peak flow spectrum were observed in both groups(P<0.05),and the study group had greater improvements(P<0.05).Levels of hs-CRP and UA decreased in both groups after treatment(P<0.05),with the study group showing a more significant reduction(P<0.05).The overall clinical effective rate in the study group was 95.56%(43/45),higher than 82.22%(37/45)in the control group(χ^(2)=4.050,P<0.05).No serious adverse reactions were reported in either group during treatment.Conclusion Xiaoshuan Zhitong Capsules combined with Buyang Huanwu Decoction effectively improved hemodynamics,regulated metabolic responses,and suppressed inflammatory reactions in patients with arteriosclerosis obliterans of limbs,demonstrating superior clinical efficacy and high safety.
