摘  要:目的评估盾形术前设计的毛囊单位提取(follicular unit extraction,FUE)技术治疗男性大面积雄激素性脱发的有效性和安全性。方法自2020年5月至2022年8月,上海交通大学医学院附属第九人民医院整形外科对61例NorwoodHamiltonⅤ-Ⅵ级的男性雄激素性脱发患者,进行盾形术前设计的FUE技术治疗。观察患者的一般状况、手术信息、毛发自然度、患者满意度和不良反应。结果61例患者平均移植(3799±444)FUs,单根移植密度为60 FUs/cm^(2),多根移植密度为30~50 FUs/cm^(2),平均手术时间为(9.5±1.0)h。李克特(Likert)量表评分结果:患者对毛发自然度和手术满意度的平均评分分别为4.90分和4.74分;医师对毛发自然度和手术满意的平均评分分别为4.87分和4.82分。术后随访6~12个月,无严重并发症发生。结论采用盾形设计的FUE技术治疗男性大面积雄激素性脱发,术后移植毛发具有相对自然的密度与外观,患者满意度较高,而且术后并发症较少,值得临床推广应用。Objective To assess the effectiveness and safety of follicular unit extraction(FUE)with shield-shaped preoperative design in treating advanced androgenetic alopecia in men.Methods Sixty-one male patients with Norwood-Hamilton grades V-VI androgenetic alopecia were treated with FUE involving shield-shaped preoperative planning.Observations included general patient conditions,surgical details,naturalness of hair,patient satisfaction,and adverse reactions.Results The 61 patients had an average of(3799±444)FUs transplanted,with a single graft density of 60 FUs/cm^(2),and a multi-graft density of 30~50 FUs/cm^(2).The average duration o surgery was(9.5±1.0)h.Patients rated the naturalness of their hair at 4.90 on the Likert scale and their satisfaction at 4.74.Doctors rated the naturalness of their hair at 4.87 on the Likert scale and their satisfaction at 4.82.All patients were followed up for 6 to 12 months,no severe side effects were observed.Conclusion The use of the shield pre-designed FUE technique for treating extensive androgenetic alopecia in males results in transplanted hair that appears relatively natural in density and appearance.The technique is associated with high patient satisfaction and few postoperative complications,making it worthy of clinical application and promotion.
