摘 要:目的 分析北京市通州区2011-2017年布鲁菌病(简称“布病”)的发病特征,为防控工作提供理论依据。方法 采用描述流行病学方法,分析2011-2017年资料,并与2018-2020年比较,定性资料的比较采用χ2检验。结果 2011-2017年报告布鲁菌病病例104例,无死亡病例,发病率为0.25/10万~2.38/10万,呈上升趋势,不同年份发病率差异有统计学意义(χ^(2)=34.82,P<0.01)。发病以农民为主,占46.15%;3~8月为发病高峰,占71.15%;病例集中在40~<70岁年龄组,占76.92%,男女性别比为4.78∶1;77.88%的病例与羊有接触史,重点人群血清学平均阳性率为0.63%,无防护措施的病例占80.68%。2018-2020年报告布病病例53例,发病率为0.61/10万~1.86/10万,呈逐年下降趋势,三间分布特征和感染来源等特征相似,无防护措施的病例占50.00%。结论 北京市通州区布病以男性、农民和养羊人群为主,2011-2017年发病率呈上升趋势,2018-2020年呈逐年下降趋势,无防护措施的病例占比下降,应继续加强联防联控和健康教育等工作。Objective To analyze the characteristics of brucellosis in Tongzhou District of Beijing from 2011 to 2017,so as to provide theoretical basis for prevention and control.Methods Descriptive epidemiological methods were used to analyze data from 2011 to 2017,and compared with those from 2018 to 2020.Chi-square test was used for qualitative data comparison.Results A total of 104 brucellosis cases were reported from 2011 to 2017,no dead case,incidence rate was 0.25/100000 to 2.38/100000,showed a upward trend.There was a statistically significant difference in incidence rates between different years(x^(2)=34.82,P<0.01).The incidence of the disease was dominated by farmers,accounting for 46.15%.The incidence peak was from March to August,accounting for 71.15%.Cases were concentrated in the age group of 40 to<70 years,accounting for 76.92%,the gender ratio of male-to-female was 4.78:1.There were 77.88%cases which had contact history with sheep.The average serological positive rate of key groups was 0.63%.Cases without protective measures accounted for 80.68%.A total of 53 brucellosis cases were reported from 2018 to 2020,with incidence rates ranged from 0.61/100000 to 1.86/100000,and the trend was decreased year by year.The characteristics on spatial,temporal and population distribution and infection sources were similar.Cases without protective measures accounted for 50.00%.Conclusion The main populations with brucellosis in Tongzhou District of Beijing are male,farmers,and sheep farmers.The incidence rate shows an upward trend from 2011 to 2017,and a downward trend from 2018 to 2020.The proportion of cases without protective measures are decreased.It is suggested that joint prevention and control and health education should be strengthened.
