摘　　要：产程管理是产房工作的重要内容,胎方位和胎姿势异常是导致头位难产的首要原因,增加围产期并发症发生风险。目前,临床上主要通过阴道指诊和肛门指诊了解子宫颈口扩张、胎方位、胎先露位置及产道等情况,以指导产程管理。但其主观性强且操作者之间存在个体差异,胎方位和胎姿势评估不准确可能会增加阴道手术助产困难或失败的风险。产时超声检查可用于监测子宫颈口扩张程度、胎先露位置、胎方位等,更客观、准确且能避免阴道检查不适及感染风险,并具有良好的重复性,有助于辅助产程管理决策。Labor management is a crucial aspect of work within the delivery room.Fetal malpositions and cephalic malpresentations are well-known factors that contribute to a lack of progress during labor,which in turn,increases the risk of peripartum complications.Obstetricians traditionally utilize vaginal and rectal examination to monitor the progress of labor and assess fetal position,fetal head station and the birth canal,which guides labor management.However,these traditional methods have been found to be highly subjective,and can result in significant interoperator variability.The inaccurate assessment of the fetal head position and station may increase the risk of difficult or failed assisted vaginal delivery.Intrapartum ultrasound is used to monitor various factors during labor,such as cervical dilatation,fetal head position,and station.This technique offers greater objectivity,accuracy,and avoids discomfort and infection risks associated with vaginal examinations.It is of good repeatability and can be beneficial in the management of the labor process.
