摘　　要：目的评估老年人群肌少症不同筛查量表的应用价值。方法2021年11月至2022年5月于社区招募年龄在60岁及以上老年人512例,测量患者握力、步速、骨骼肌质量指数等指标,按照亚洲肌少症工作组建议的诊断标准进行肌少症诊断。通过问卷方式由研究者对研究对象开展一对一的访谈,内容包括一般情况:出生日期、年龄、身高、体质量等;采用SARC-F问卷评分量表、SARC-Calf评分量表、MSRA-5、MSRA-7问卷等评估受试者的情况。采用受试者操作特征曲线(receiver operating characteristic curve,ROC)比较不同筛查量表对肌少症的应用价值。结果共入组512例老年受试者,按照亚洲肌少症工作组诊断标准,将入组老年人群分为肌少症组和非肌少症组。肌少症患病率为5.1%。通过ROC曲线评估,SARC-F量表对肌少症发生具有较好的识别能力,曲线下面积(area under the curve,AUC)为0.739,敏感性为0.750,特异性为0.716。SARC-Calf评分量表较SARC-F问卷评分量表具更高的应用价值,其AUC为0.817,敏感性为0.958,特异性为0.587。结论社区招募的老年人群的肌少症患病率为5.1%。在临床研究中,SARC-Calf评分量表是筛查肌少症简便易行的方法,具有较好的敏感性和特异性,可用于大规模人群肌少症筛查。Objective To evaluate the application value of different sarcopenia screening scales in the elderly population.Methods From November 2021 to May 2022,512 elderly people aged 60 and above in the community were recruited.Grip strength,walking speed,skeletal muscle mass index,and other indicators were measured,and sarcopenia was diagnosed according to the criteria recommended by the Asian Sarcopenia Working Group(AWGS).The researchers conducted one-to-one interviews with the subjects through questionnaires.The contents included general information:date of birth,age,height,weight,etc.SARC-F scale,SARC-Calf scale,MSRA-5 and MSRA-7 questionnaire were used to evaluate the skeletal muscle condition of the subjects.Receiver operating characteristic curves(ROC)were applied to evaluate the application value of different screening scale for sarcopenia.Results A total of 512 elderly subjects were enrolled and divided into sarcopenia group and non-sarcopenia group.The prevalence of sarcopenia was 5.1%.By ROC curve evaluation,SARC-F and SARC-Calf scales had good application value for sarcopenia.The area under the curve(AUC)of the SARC-Calf scale was 0.817,the sensitivity was 0.958,and the specificity was 0.578.Conclusions The prevalence of sarcopenia was 5.1%in the elderly population recruited in the community.In clinical studies,SARC-Calf was demonstrated to be a a simple and easy screening criteria for sarcopenia,with good sensitivity and specificity,and can be used to screen for sarcopenia in large populations.
