摘　　要：目的探究自体毛发移植技术在儿童瘢痕性秃发、秃眉中的疗效和经验。方法收集了北京儿童医院烧伤整形外科2019年1月至2021年2月收治的因手术、烧伤、外伤导致的瘢痕性秃发、秃眉患儿共13例,其中男10例,女3例;年龄为(7.0±3.0)岁,年龄范围是4~12岁;其中10例秃发,3例秃眉。根据患儿病情特点采取自体毛囊单位提取(follicular unit extraction,FUE)或毛囊单位头皮条切取(follicular unit transplantation,FUT)移植术,随访时计算成活率、毛发生长情况、并发症,并以问卷的形式按照非常满意、满意、不满意3个标准对患儿家属进行满意度评价。结果 13例中有8例采用FUE、4例采用FUT、1例采用FUT+FUE的治疗方式。术后随访6~12个月,13例患儿毛发成活率平均值为80.9%(植发77.8%,植眉91.5%),临床效果非常满意者4例,基本满意者7例,2例认为疗效不满意,且已实施完成二次加密术。所有患儿均无术后并发症,移植毛发接近自然毛发分布特点,且供区瘢痕不明显。结论自体毛发移植技术是治疗儿童瘢痕性秃发、秃眉的安全有效的手段,应当临床推广。Objective To explore the clinical characteristics of autologous hair transplantation in children with scarring alopecia and eyebrow defect.Methods From January 2019 to February 2021,retrospective review was conducted for 13 children with scarring alopecia and eyebrow defect.A total of 13 autologous hair transplantations were performed for scalp(n=10)and eyebrow(n=3).There were 10 boys and 3 girls with an average age of(7±3)(4-12)year.Follicular unit extraction(FUE,n=8),follicular unit transplantation(FUT,n=4)and FUE+FUT(n=1)were performed according to individual characteristics.Treatment processes consisted of preoperative preparation,design,hair follicle extraction,preparation and hair implantation.Survival rate of hair,hair growth and complications were recorded during follow-ups.The subjective outcome of children's family members was evaluated in the form of questionnaire of 3 levels:very satisfied,satisfied and dissatisfied.Results After a postoperative follow-up period of(6-12)month,average hair survival rate was 80.9%(77.8%scalp,91.5%eyebrow).Four cases(30.8%)were very satisfied,7(53.8%)satisfied and 2(15.4%)dissatisfied.Unsatisfactory children underwent secondary hair transplantation due to a low hair survival rate.Transplanted hairs grew naturally and scars in donor area were not obvious.None had serious complications.Conclusions Autologous hair transplantation is both safe and efficacious for scarring alopecia and eyebrow defects in children.And it is worthy of clinical popularization.
