健心胶囊联合胺碘酮治疗慢性心力衰竭并室性心律失常临床研究

摘　　要：目的观察健心胶囊联合胺碘酮治疗慢性心力衰竭并室性心律失常(CHFVA)的疗效。方法将我院70例CHFVA患者随机分为治疗组与对照组各35例。对照组服用胺碘酮,治疗组服用健心胶囊联合胺碘酮。比较两组临床疗效、心率(HR)、PR间期、校正QT间期(QTc)、QT离散度(QTd),左心室舒张末径(LVDD)、左心室射血分数(LVEF)、N末端B型钠尿肽前体(NT-proBNP),24 h相邻正常R-R间期标准差(SDNN)、每5 min正常R-R间期平均值标准差(SDANN)、相邻正常R-R间期差值均方差(rMSSD),室性早搏次数。结果治疗后,治疗组总有效率高于对照组(P<0.05);两组PR间期高于治疗前,HR、QTc、QTd低于治疗前,治疗组PR间期高于对照组,HR、QTc、QTd低于对照组(P<0.05);两组LVEF高于治疗前,LVDD、NT-proBNP低于治疗前,治疗组LVEF高于对照组,LVDD、NT-proBNP低于对照组(P<0.05);两组SDDN、SDANN、rMSSD高于治疗前,治疗组SDDN、SDANN、rMSSD高于对照组(P<0.05);两组室性早搏次数低于治疗前,治疗组室性早搏次数低于对照组(P<0.05)。结论健心胶囊联合胺碘酮治疗CHFVA能获得更好的疗效,可提高心功能,改善心电活动,减少室性早搏次数。Objective To observe the therapeutic effect of Jianxin Capsules combined with amiodarone on chronic heart failure complicated with ventricular arrhythmias(CHFVA).Methods Seventy CHFVA patients were randomly divided into treatment group and control group,with 35 cases in each group.Control group received amiodarone,while treatment group received Jianxin Capsules on the basis of control group.The efficacy,heart rate(HR),PR interval,corrected QT interval(QTc),QT dispersion(QTd),left ventricular end diastolic diameter(LVDD),left ventricular ejection fraction(LVEF),Nterminal B-type natriuretic peptide precursor(NT pro BNP),24-hour standard deviation of adjacent normal R-R interval(SDNN),standard deviation of mean normal R-R interval every 5 minutes(SDANN),mean square deviation of difference between adjacent normal R-R intervals(r MSSD),and number of ventricular premature beats were compared between the two groups.Results After treatment,the total effective rate in treatment group was higher than that in control group(P<0.05);the PR interval of both groups was higher than before treatment,and was higher in treatment group than control group;HR,QTc,and QTd were lower than before treatment,and were lower in treatment group than control group(P<0.05);the LVEF of both groups was higher than before treatment,and was higher in treatment group than control group;LVDD and NT pro BNP were lower than before treatment,and were lower in treatment group than control group(P<0.05);the SDDN,SDANN,and r MSSD of both groups were higher than before treatment,and were higher in treatment group than control group(P<0.05);the number of ventricular premature beats in both groups was lower than before treatment,and were lower in treatment group than control group(P<0.05).Conclusion The combination of Jianxin Capsules and amiodarone has better efficacy in treatment of CHFVA,improving cardiac function and cardiac activity,reducing the frequency of ventricular premature beats.
