消肿止痛膏治疗Ⅲ、Ⅳ度踝关节骨折患者术前红肿热痛临床疗效及对炎症指标的影响

摘　　要：目的观察消肿止痛膏治疗Ⅲ、Ⅳ度踝关节骨折患者术前关节局部红肿热痛疗效及对炎症指标白细胞计数(WBC)、降钙素原(PCT)、超敏C反应蛋白(hs-CRP)、血沉(ESR)水平、D二聚体(D-D)和术后伤口愈合的影响。方法将我院收治的60例踝关节骨折患者采用随机数字表法分为对照组与中药组各30例。对照组给予常规治疗,中药组在对照组治疗基础上联合消肿止痛膏治疗,对比两组患者关节局部症状及治疗前后炎症指标、血浆WBC、PCT、hs-CRP、ESR、D-D水平变化以及术后伤口愈合情况。结果治疗后,中药组关节局部症状红肿热痛均轻于对照组(P<0.05);两组WBC、PCT、hs-CRP、ESR、D-D水平均低于治疗前(P<0.05),且中药组WBC、PCT、hs-CRP、ESR、D-D水平低于对照组(P<0.05);中药组手术后伤口愈合时间短于对照组,且伤口不良反应发生率均低于对照组(P<0.05)。结论消肿止痛膏治疗踝关节骨折能够改善患者局部关节症状,减轻炎症反应,促进术后手术伤口愈合,安全性高,值得推广。Objective To observe the effects of Xiaozhong Zhitong Ointment on preoperativeⅢ,Ⅳankle fracturejoint redness,swelling,heat and pain,and inflammatory index levels[white blood cell count(WBC),procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP),erythrocyte sedimentation rate(ESR)levels,D-dimer(D-D)].Methods Sixty patients with ankle fracture were randomly divided into control group and TCM group,30 cases each.Control group was given routine treatment,and TCM group was treated with Xiaozhong Zhitong Ointment on the basis of control group.The local symptoms of the joints,inflammatory indexes(WBC,PCT,hs-CRP,ESR and D-D)before and after treatment,and postoperative wound healing were compared between the two groups.Results After treatment,the local symptoms of redness,swelling,heat and pain in TCM group were lighter than those in control group(P<0.05);the levels of WBC,PCT,hs-CRP,ESR and D-D in both groups were lower than those before treatment(P<0.05),and were lower in TCM group than control group(P<0.05);the wound healing in TCM group was faster than that in control group,and the incidence of wound adverse reactions was lower than that in control group(P<0.05).Conclusion Xiaozhong Zhitong Ointment can improve local joint symptoms,reduce inflammatory reaction,and promote postoperative wound healing in treatmont of ankle fracture.It has high safety and is worthy of promotion.
