电针联合芪藤软坚散治疗急性闭合性跟腱断裂术后疗效分析

摘　　要：目的观察电针联合芪藤软坚散治疗急性闭合性跟腱断裂术后(AACATRS)的疗效。方法将我院70例AACATRS患者随机分为治疗组与对照组各35例。对照组用常规康复训练治疗,治疗组在常规康复训练的基础上用电针联合芪藤软坚散治疗。比较两组优良率,在术后6周、24周分别采集等速肌力测试指标[峰力矩(PFPT)、峰力矩/体质量(PT/BW)、总功(TW)]、美国足与踝关节协会(AO-FAS)评分、踝关节活动度、视觉疼痛模拟评分(VAS)、小腿周径差、Berg平衡量表评分、跖屈肌力分级。比较两组术后24周内并发症发生率。结果术后24周,治疗组优良率高于对照组(P<0.05);两组PFPT、PT/BW、TW、AOFAS评分、踝关节活动度、Berg平衡量表评分高于术后6周,治疗组PFPT、PT/BW、TW、AO-FAS评分、踝关节活动度、Berg平衡量表评分高于对照组(P<0.05);两组VAS、小腿周径差低于术后6周,治疗组VAS、小腿周径差低于对照组(P<0.05);两组跖屈肌力分级优于术后6周,治疗组跖屈肌力分级优于对照组(P<0.05);治疗组并发症发生率低于对照组(P<0.05)。结论电针联联合芪藤软坚散可明显改善AACATRS患者症状,提高肌力,增加踝关节活动度,减轻疼痛和肿胀,恢复平衡,疗效较优。Objective To analyze the therapeutic effect of electroacupuncture and Qiteng Ruanjian Powder combined therapy on postoperative acute closed Achilles tendon rupture(AACATRS).Methods Seventy AACATRS patients were randomly divided into treatment group and control group,with 35 cases in each group.Control group was treated with routine rehabilitation training,while treatment group was treated with electroacupuncture combined with Qiteng Ruanjian Powder on the basis of control group.Compare the excellent rates of two groups,and collect isokinetic muscle strength test indicators[peak torque(PFPT),peak torque/body mass(PT/BW),total work(TW)],American Association of Foot and Ankle(AO-FAS)score,ankle range of motion,Visual Analogue Scale(VAS)score,calf circumference difference,Berg Balance Scale score,and plantar flexor muscle strength grading at 6 and 24 weeks after surgery,compare the incidence of complications within 24weeks after surgery between the two groups.Results The excellent and good rate of treatment group was higher than that of control group(P<0.05);at 24 weeks post surgery,the PFPT,PT/BW,TW,AO-FAS scores,ankle range of motion,and Berg Balance Scale scores in both groups were higher than those at 6 weeks post surgery;and were higher in treatment group than control group(P<0.05);at 24 weeks post surgery,the VAS score and leg circumference of both groups were lower than those at 6 weeks post surgery,and were lower in treatment group than control group(P<0.05);at 24 weeks post surgery,the plantar flexor muscle strength grading in both groups was better than those at 6 weeks post surgery,and was better in treatment group than control group(P<0.05);the incidence of complications within 24 weeks after surgery in treatment group was lower than that in control group(P<0.05).Conclusion The combination therapy of electroacupuncture and Qiteng Ruanjian Powder significantly improves the symptoms of AACATRS patients,enhances muscle strength,increases ankle joint range of motion,reduces pain and swelling,restores balance,
