砭石灸联合中药膏摩治疗老年糖尿病下肢神经病变的效果观察

摘　　要：目的研究砭石灸联合中药膏摩治疗老年糖尿病下肢神经病变的治疗效果。方法将2021年9月至2022年9月我院内分泌科收治的老年糖尿病并伴下肢神经病变的患者作为研究对象,按随机数字表法分为对照组和试验组,每组各30例。对照组采用常规治疗方案(控制血糖加硫辛酸和甲钴胺药物)治疗,治疗组在常规治疗的基础上采用砭石灸联合中药膏摩中医外治治疗,1周为一个疗程,一疗程后观察疗效,比较两组患者中医证候疗效、正中神经和绯总神经的运动和感觉传导速度和TCSS评分。结果治疗组总有效率明显高于对照组,两组疗效对比,差异有统计学意义(P<0.05);治疗组正中神经、腓总神经MNCV和SNCV提高优于对照组,差异有统计学意义(P<0.05);治疗组多伦多临床评分系统(TCSS)评分明显低于对照组,差异有统计学意义(P<0.05)。结论砭石灸联合中药膏摩治疗老年糖尿病下肢神经病变具有温通经络、活血化瘀、理气活血、散寒祛风,达到营养神经、改善微循环的作用,疗效优于西医常规治疗。Objective To study the therapeutic effect of stone needle moxibustion combined with Chinese ointment on senile diabetic lower extremity neuropathy.Methods Elderly patients with diabetes mellitus and lower extremity neuropathy admitted to the Endocrinology Department of Beijing Daxing District Integrated Traditional Chinese and Western Medicine Hospital from September 2021 to September 2022 were selected as the study objects,and were divided into the control group and the experimental group with 30 cases each according to random number table method.The control group was treated with conventional treatment(blood glucose control plus Lipoic acid and Mecobalamine).On the basis of conventional treatment,the treatment group was treated with TCM external treatment of stone needle and moxibustion combined with Chinese ointment and moxibustion for one week.The curative effect was observed after one course of treatment.The curative effect of TCM syndrome,the motor and sensory conduction speed of median and common nerve,and the TCSS score of the two groups were compared.Results The total effective rate of the treatment group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,MNCV and SNCV of median nerve and common peroneal nerve in the treatment group were improved better than those in the control group,and the difference was statistically significant(P<0.05).The TCSS score after treatment was significantly lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Stone needle moxibustion combined with Chinese ointment in the treatment of senile diabetic lower extremity neuropathy has the following effects:warming channels,promoting blood circulation and removing blood stasis,regulating qi and promoting blood circulation,dispelling cold and dispelling wind,nourishing nerves and improving microcirculation.
