摘　　要：目的 探讨药师应用PDCA循环法提高重点监控药品合理使用率的实践及成效。方法 采用PDCA循环法,通过现状分析、原因解析,明确重点监控药品不合理使用7项问题真因。从构建重点监控药品管理制度体系、优化病例筛选-处方点评-临床反馈流程、开展合理用药培训与督导、依托信息系统四方面制定针对性方策予以改进。结果 重点监控药品管理流程更加规范,重点监控药品合理使用率由86.94%提升到96.13%,并持续上升。结论 药师应用PDCA循环法参与重点监控药品管理,可有效提高重点监控药品合理使用率,保障了患者用药安全,践行了优质药学服务理念。OBJECTIVE To explore the practice and effectiveness of pharmacists in improving rational utilization rate of key monitored drugs based on PDCA cycle method.METHODS PDCA cycle method was adopted to identify the real causes of 7 issues of irrational use of key monitored drugs through current situation analysis and cause analysis.Targeted strategies were developed to improve the management system of key monitored drugs by building the management system of key monitored drugs,optimizing the process of case screening-prescription comment-clinical feedback,carrying out training and supervision on rational drug use,and relying on information system.RESULTS The management process of key monitored drugs was more standardized.The rational utilization rate of key monitored drugs increased from 86.94% to 96.13%,and continued to rise.CONCLUSION Pharmacists participating in the management of key monitored drugs based on PDCA cycle can effectively improve the rational utilization rate of key monitored drugs,ensure the drug safety of patients,and practice the concept of high-quality pharmaceutical care.
