温阳散结方穴位贴敷联合脐火疗法改善癌性腹水患者生活质量及癌因性疲乏的研究

摘　　要：目的观察温阳散结方穴位贴敷联合脐火疗法治疗癌性腹水患者的临床疗效,客观评价穴位贴敷联合脐火疗法治疗癌性腹水的疗效及可行性和安全性,以期更好地指导其临床实践。方法采用随机对照的研究方法,把符合纳排标准的63例研究对象随机分为两组,对照组31例患者给予西医利水治疗,治疗组32例患者给予温阳散结方穴位贴敷联合脐火疗法。穴位贴敷和脐火疗法由中医特色治疗室开展操作,观察指标及实验数据由专属科研质控人员进行观察记录。主要观察指标为癌性腹水患者病性证素分布特点、腹水近期疗效、疾病控制率(Disease Control Rate,DCR)、癌因性疲乏改善情况;次要观察指标为不良反应。结果癌性腹水患者病性为虚,以气虚和阳虚为主;治疗组在疾病控制率、癌因性疲乏程度改善方面均明显高于对照组,两组差异均具有统计学意义(P<0.05)。两组均未出现胃肠道反应、皮肤过敏反应、肝肾功能损伤,两组不良事件率无统计学差异。结论在中医扶正抑瘤理论指导下,温阳散结方穴位贴敷联合脐火疗法中医特色治疗开展过程中,能更好的改善癌性腹水患者消化道症状,提高生活质量,改善癌因性疲乏情况,患者满意度高,安全性良好。中医外治法联合治疗方案用于癌性腹水患者安全有效,可为中医外治法联合方案提供临床思路。Objective To observe the clinical efficacy of Wenyang Sanjie prescription acupoint application combined with umbilical fire therapy in the treatment of cancerous ascites,and objectively evaluate the efficacy,feasibility and safety of acupoint application combined with umbilical fire therapy in the treatment of cancerous ascites,in order to bet⁃ter guide its clinical practice.Methods Using the method of random controlled study,63 subjects who met the standard of Nanpai were randomly divided into two groups.31 patients in the control group were treated with western medicine di⁃uresis and 32 patients in the treatment group were treated with Wenyang Sanjie prescription acupoint application com⁃bined with umbilical fire therapy.Acupoint application and umbilical fire therapy are operated by the characteristic treat⁃ment room of traditional Chinese medicine,and the observation indexes and experimental data are observed and recorded by exclusive scientific research quality control personnel.The main observation indicators were the distribution character⁃istics of disease syndrome elements,short-term curative effect of ascites,disease control rate(DiseaseControlRate,DCR)and improvement of cancer-related fatigue in patients with cancerous ascites,while the secondary observation in⁃dex was toxic and side effects.Results The disease of patients with cancerous ascites was deficiency,mainly qi deficiency and yang deficiency,and the disease control rate and the improvement of cancer-related fatigue in the treat⁃ment group were significantly higher than those in the control group,and there were significant differences between the two groups(P<0.05).There was no gastrointestinal reac⁃tion,skin allergic reaction,liver and kidney function injury in the two groups,and there was no significant difference in the rate of adverse events between the two groups.Conclusion Under the guidance of traditional Chinese medicine Fuzheng anti-tumor theory,Wenyang Sanjie prescription acupoint application combined with umbilical fi
