产前超声检查对中晚期孕妇胎儿血管前置的诊断价值

摘　　要：目的探讨产前超声检查对中晚期孕妇胎儿血管前置的诊断价值.方法选取江西省妇幼保健院2018年1月-2021年6月疑似胎儿血管前置的孕妇62例,全部孕妇均在孕中晚期(13-40周)接受超声检查,并随访至产妇妊娠结束,了解其妊娠结局情况,将胎儿娩出后病理结果作为参照依据,分析产前超声检查诊断孕妇胎儿血管前置的效能.结果62例疑似胎儿血管前置孕妇均行剖宫产分娩,经病理检查结果得知,有41例确诊胎儿血管前置,占66.13%;参照病理检查结果,产前超声检查诊断中晚期孕妇胎儿血管前置的灵敏度、特异度、准确度分别为95.12%(39/41)、85.71%(18/21)、91.94%(57/62),且产前超声检查与病理检查诊断结果有极好的一致性(Kappa=0.818).结论产前超声检查诊断中晚期孕妇胎儿血管前置有较高价值,值得临床推广应用.Objective To explore the diagnostic value of prenatal ultrasonography in the diagnosis of fetal vascular previa in middle and late pregnant women.Methods A total of 62 pregnant women suspected of fetal vascular previa were selected from Jiangxi Maternal and Child Health Hospital from January 2018 to June 2021.All pregnant women received ultrasound examination in the middle and third trimester(13-40 weeks),and were followed up until the end of pregnancy to understand their pregnancy outcomes,and the pathological results after delivery of the fetus were taken as reference.To analyze the efficacy of prenatal ultrasonography in diagnosing fetal blood vessel preposition in pregnant women.Results All 62 suspected fetal vascular previa were delivered by cesarean section.41 cases(66.13%)were confirmed by pathological examination.According to the results of pathological examination,the sensitivity,specificity and accuracy of prenatal ultrasonography in the diagnosis of fetal blood vessel preposition were 95.12%(39/41),85.71%(18/21)and 91.94%(57/62),respectively.The results of prenatal ultrasonography and pathological examination showed a good consistency(Kappa=0.818).Conc lusion Prenatal ultrasonography has high value in the diagnosis of fetal vascular previa in middle and late pregnant women and is worthy of clinical application.
