宁夏居民就医获得感现状及影响因素研究

摘　　要：目的:通过对宁夏居民就医获得感现状进行调查,分析宁夏居民就医获得感影响因素,以期为宁夏居民就医获得感的提升提供决策参考,为进一步深化医药卫生事业改革提供参考。方法:2019年11—12月,采用问卷调查的方式对3家宁夏样本医院门诊的600例患者进行调查,采用多重回归分析探索宁夏居民就医获得感的影响因素。结果:宁夏居民就医获得感总体得分为(38.90±9.06)分,就医获得感各维度得分排序分别为就医安全获得感、就医有效获得感、就医价廉获得感、就医方便获得感。影响宁夏居民就医获得感的主要因素分别为文化程度、医保类型、家庭年收入、就诊医生级别、是否下级医疗机构转诊(P<0.05)。结论:宁夏居民就医获得感处于中上水平,但仍有上升空间;相关部门应加强医联体建设,促进优质资源下沉;搭建信息化平台,优化服务流程;提升基层服务能力,发挥医保杠杆作用,引导患者合理就医;通过培养高水平卫生人才等手段提升居民就医获得感。OBJECTIVE Through a survey of sense of gain in medical treatment in Ningxia,the current situation of sense of gain in medical treatment was analyzed and elements explored,so as to propose suggestions for enhancing the sense of gain in medical treatment and deepening medical reform.METHODS From November to December 2019,a questionnaire survey was conducted among outpatients in sample hospitals in Ningxia,and multiple regression analysis was used to explore the influencing factors of Ningxia residents’sense of access to medical treatment.RESULTS The overall score of the sense of gain in medical treatment was(38.90±9.06).The scores of each dimension of the sense of medical access were ranked as the sense of medical safety,the sense of effective medical access,the sense of cheap medical access and the sense of convenient medical access,education level,common types of medical insurance,annual family income,the title of consultation doctors,referral from lower level medical institutions or not and the personal psychological comparison tendency were the Influencing factors of the Sense of Gain in Medical Treatment(P<0.05).CONCLUSION There is still room for improvement though the sense of the gain in medical treatment was in the upper middle level.Relevant departments should promote the sinking of high-quality resources by strengthening the construction of medical Con⁃sortium,build an information platform and optimize the service process,improve the grass-roots service capacity,give play to the le⁃verage of medical insurance,and guide patients to seek medical treatment reasonably.Cultivate high-level health talents and other means to improve residents’sense of medical access.
