摘  要:目的:探究早期下床活动联合呼吸功能训练对老年冠脉搭桥术后患者血管动力学、运动能力及呼吸功能的影响。方法:选取2017年10月—2020年8月平顶山市第一人民医院收治的121例老年冠脉搭桥术后患者作为研究对象,采用随机数字表法将其分成对照组(n=60)和研究组(n=61)。对照组实施常规护理及呼吸功能训练,观察组在此基础上增加早期下床活动干预,对比两组血管动力学指标、运动功能、呼吸功能及不良事件。结果:干预后,观察组射血分数(EF)水平高于对照组,左室舒张末期直径(LVEDD)水平低于对照组;观察组Rivermead运动指数评分、6 min步行距离均高于对照组;观察组动脉血氧分压(PaO_(2))、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、深吸气量(IC)均高于对照组;观察组不良事件的发生低于对照组,差异均有统计学意义(P值均<0.05)。结论:早期下床活动联合呼吸功能训练干预冠脉搭桥术后患者,可增加患者肺通气能力,改善运动功能,减少不良事件的发生。OBJECTIVE To explore the effects of early getting out of bed activity combined with respiratory function training on vascular dynamics,motor ability and respiratory function in elderly patients after coronary artery bypass grafting.METHODS 121 el⁃derly patients after coronary artery bypass grafting in our hospital were selected and divided into two groups by random number table method.The control group(60 cases)received routine nursing and respiratory function training,and the observation group(61 cases)received early intervention to get out of bed.The vascular dynamics indexes,motor function,respiratory function and ad⁃verse events were compared between the two groups.RESULTS After intervention,the ejection fraction(EF)level of observation group was higher than that of control group,and the LVEDD level of left ventricle was lower than that of control group.Rivermead motor index score and 6 minutes walking distance in observation group were higher than those in control group.The partial pressure of arterial oxygen(PaO2),forced vital capacity(FVC),forced expiratory volume in the first second(FEV1)and deep inspiratory volume(IC)in the observation group were higher than those in the control group.The incidence of adverse events in the observa⁃tion group was lower than that in the control group(P<0.05).CONCLUSION Early getting out of bed activity combined with respi⁃ratory function training can increase lung ventilation capacity,improve motor function and reduce the occurrence of adverse events after coronary artery bypass grafting.
