细胞角蛋白19半乳糖凝集素3和间皮表面微绒毛抗原在甲状腺乳头状癌组织中的表达及其与临床病理特征和预后的相关性

摘　　要：目的 探讨细胞角蛋白19(cytokeratin 19,CK19)、半乳糖凝集素3(galectin-3,Gal-3)、间皮表面微绒毛抗原(mesothelial surface microvilli antigen,MC)在甲状腺乳头状癌组织中的表达水平及与临床病理特征和预后的相关性。方法 选择2019年1月—2020年1月遵义市第一人民医院收治的122例甲状腺乳头状癌患者作为研究对象,分别采集每位患者的甲状腺乳头状癌组织和癌旁组织,采用免疫组化法检测各组织标本的CK19、Gal-3、MC阳性率,并分析CK19、Gal-3、MC表达与甲状腺乳头状癌临床病理特征和预后之间的关系。结果 甲状腺乳头状癌组织的CK19、Gal-3、MC阳性表达率均高于癌旁组织,差异有统计学意义(P<0.05)。甲状腺乳头状癌CK19、Gal-3、MC的阳性表达在不同分期、不同分化程度、有无淋巴结转移患者中差异有统计学意义(P<0.05)。经术后3年随访,CK19、Gal-3、MC阳性表达组患者复发率高于阴性表达组,差异有统计学意义(P<0.05)。结论 CK19、Gal-3、MC在甲状腺乳头状癌中高表达,可能在肿瘤浸润、转移中发挥协同作用,并且与预后密切相关。Objective To investigate the expression of cytokeratin 19(CK19),galactose agglutinin 3(Galectin-3),and mesothelial surface microvilli antigen(MC)in papillary thyroid cancer tissue and their correlation with clinical pathological characteristics and prognosis.Methods A total of 122 patients with papillary thyroid cancer admitted to the First People's Hospital of Zunyi City from January 2019 to January 2020 were selected as the subjects.Thyroid papillary cancer tissue and adjacent normal tissue were collected from each patient.Immunohistochemical methods were used to detect the positive rates and expression levels of CK19,Gal-3,and MC in paired tissue samples.The relationship between CK19,Gal-3,MC and the clinical-pathological characteristics,prognosis of papillary thyroid cancer were analyzed.Results The positive expression rates of CK19,Gal-3,and MC in papillary thyroid cancer tissue were significantly higher than those in adjacent normal tissue,(P<0.05).There were significant differences in the expression rates of CK19,Gal-3,and MC indifferent stages,differentiation levels,and lymph node metastasis(P<0.05).After 3 years of follow-up after surgery,the recurrence rate of CK19,Gal-3,and MC positive expression groups was significantly higher than that of the negative expression group(P<0.05).Conclusion CK19,Gal-3,and MC are highly expressed in papillary thyroid carcinoma,which may play a synergistic role in tumor infiltration and metastasis,and are closely related to prognosis.
